Question:
I am frty yrs. old and have asthma. I take accolate twice daily and use the inhalers albuterol, flovent, and combivent. My mother has same symptoms as I do, but her Doctor says hers is Emphysema she is sixty yrs old. They have told her adults dont get asthma, they get emphysema. My question is whats the difference in the two and why is it treated the same way.
Asthma is an inflammation-induced bronchoconstriction. Treatment of the bronchospasm with short- and long-acting bronchodilators, and the underlying inflammation with inhaled steroids can quell the inflammation; and permit an asthmatic to lead a near-normal life. Emphysema [which can result from long-untreated asthma] is a permanent loss of elasticity in the aveoli. They are treated the same because many of the same drugs will assist both conditions. Adults do get asthma, called adult onset asthma. Emphesema is generally contracted by smokers tho there is a hereditary version. Asthma is generally considered a reversible condition with drugs. Emphesema is only slightly reversible with drugs. Many older patients have a combination of the 2. Asthma and emphesema are not treated exactly the same way, although some drugs are used for both. Accolate would only be used for asthma–it helps about 1/2 to 2/3 who try it, if it doesn’t help it should be stopped. Combivent (albuterol + Atrovent) is primarily intended for emphesema but may benefit some asthmatics. Albuterol is used for both. Flovent is used to treat the inflammation of asthmatic bronchial tubes tho some with emphysema may benefit. Ellis

We can only hope that competition will bring the benefits to our US cousins that we Europeans have. Generic Metformin, on a three doses a day basis (1500mg total dose) is sold to the British NHS at a basic price of $4 a week or thereabouts. That is the price the NHS pays; anyone not able to supply at that price does not get used. So therefore as many manufacturers are supplying at that price there is no reason why it should not be available at around those prices in the US. I have had Israeli manufactured Metformin in the past; supplied at that price. Absolutely perfect generic Metformin. The manufacturers would not be selling at the UK price if it was unprofitable. Inhaled steroids can result in localized infections with Candida albicans in the mouth and pharynx. Now that I am getting better, I have developed a very bad sore throat and my tongue feels like its on fire. If I swallow it is so sore and I have no white spots on the throat or inside of my mouth but have tiny little cheese-like particles in my mouth. Can all these medicines I am being bombarded with cause a candida yeast condition? I had a mild yeast infection before I went to the allergist, but did not associate any spreading of the condition when I was taking all this medicine. Being the "Queen Of Thrush", it sounds like that is what you have,of course the usual disclaimer (I am not a doctor….) Nystatin swish and swallow is a good start. If you still have it after that ask for Diflucan. There is also a new swish and swallow of AmphotericinB but that is when you have exhausted all other things. I’m sure that I do not have to say, rinse your mouth,blah,blah blah. Some people are just prone to thrush and you learn to deal with it. It’s pretty nasty but, brathing is better, Peace, Tish

I did have a very sore throat and the doctor lowered my dose of Azmacort and took away the atrovent and put ventolin on prn basis and I haven’t used that for about five days now. I still can’t taste anything. The pharmacist suggeste zinc tablets and I have taken those for almost a week plus used the zinc lozenges. Nothing is working. Help!!!!!!!

My wife who has asthma was told that she has to gargle right after using her inhalers to prevent yeast infection in her throat. Hope this helps. Hope you fell better soon.

If I swallow it is so sore and I have no white spots on the throat or inside of my mouth but have tiny little cheese-like particles in my mouth. Yes yes yes!!! this is almost identical to what i had two weeks ago. i was on flovent/serevent and antibiotics (doxycycline…destroyed my intestines. very sad. not doing those again) i got a sharp, painful sore throat, almost as if someone had cut a part of my throat with a knife. my tongue was painfully sore and had desnuded patches (the taste buds were gone) on about half, and white patches on the rest. i didn’t have any cheese particles but that seems to make it almost definite (as that’s the clear sign of a vaginal yeast infection!) i went in to the college health center and said "i’m on antibiotics and a steroid inhaler, i think i have a yeast infection in my mouth". the doctor looked it over, told me i was right, said she probably would have treated me for strep with more antibiotics if i hadn’t figured it out on my own (another reason to be super-informed…). she put me on nystatin liquid, 1 tsp 3 times a day for 5 days. it went away although i’m a bit nervous that it’s not completely gone. (last night i felt the sore thoat again for a bit…and i’ve lost the upper register of my voice…) the liquid was lovely, tasted like minty robitussin, but did the job. good luck! jodi

A yeast infection, AFAIK, is either caused by an alergic reaction or the yeast just finds a nice warm ear a good place to set up home. <advert I am the UK and Ireland distributor for a product called Ear Balm which is very good at clearing this up. Are you positive it’s a yeast infection? Has the dog had them in the past and this was confirmed and treated by your vet? If not, go to the vet to be sure this is what she has. I have an old (14) lhasa who has had chronic yeast infections his entire life. About 6 or 7 years ago, on one of my many trips in to see our vet, he suggested using Monostat on him, when his ears got re-infected. If you’re sexually active, your partner(s) should be treated as well — otherwise you could be getting re-infected with each act of sex. Heck, I suppose it’s even possible that you could get a penile yeast infection from fellatio performed by someone with oral thrush. : Have you tried using Tea Tree Oil? Comes in both an oil and gel : form. You can get it at any godd health food store/pharmacy. I thing : the gel like variety would be bes for yor case. I have used it for athelites : foot after every allopathic remidy failed. In days it was gone! Tea Tree : oil good for all types of topical yeast/fungus infections. It’s worth a try. Thanks, Matthew.

Advair is more expensive versus one inhaler but if you add the Flovent inhaler and the Serevent inhaler they will be less.

There are two advantages. 1) The delivery system takes less kill/practice and since MDI technique is notoriously horrible it is hoped that the delivery will be better. 2) Cost. All prices are AWP (average wholesale price). Induhvidual indusurers may make special deals. That said: Advair is more expensive that either Flovent or Serevent alone but less expensive than both. Since the Pulmicort Turbuhaler has three months worth of medicine in each device it is less expensive than Flovent and the combo od Pulmicort and Serevent is about the same as Advair. Oc course, if it cuts two co-pays to one it is cheaper for you regardless of AWP. In my experience the only insurers that won’t pay for Advair also don’t pay for Serevent. Another factor is, for me, I don’t get the 120 doses per canister of Serevent and Flovent. Theoretically the canisters should last 30 days, but I find them running low quicker than that. delivery system is different: Advair uses a powder and Flovent is a mdi. If you have problems with an mdi, then, on that basis, the switch to advair could be indicated. The problem with Advair is that during an exacerbation you cannot vary the Advair dosage, you will have to supplement with Flovent in whatever strength is required. In terms of efficacy, from personal experience, I find no difference and this is supported by my pulmonologist. The final question is cost: does your health insurance cover Advair. According to my Pulmologist the powder form is more effective in getting into your lungs and does a better job.

Only a couple of differences. — It’s a dry powder inhaler, so no propellant to react to. — It’s one puff twice daily, not 2 each twice daily (i.e., 1 not 4, twice daily) Helps compliance — There’s a counter on the device. Helps keep track of when to replace. There were studies about it being more effective than just flovent, but I haven’t seen anything about it being better than the pair of medications prescribed separately. Other than what’s listed above, it would appear to be a compliance issue. I prefer it since I cough after taking the 4 puffs, but not after taking advair. YMMV. Liam

Since I started Singulair 10mg 1x tablets 2 weeks ago, I get headaches almost daily. (Didn’t have that with Accolate). I have tried advil and tylenol but neither seems to get rid of the headache completely. Sometimes the headache is on one side only—never had that before. For those with the same sideaffect, what are you taking that seems to work on these headaches? Thanks, Lynn

If you are getting headaches from the Singulair you need to call your doctor and tell him. He may advice you to quit taking it. Any side effect of a drug should be reported to your doctor at once!! Headaches are a known side affect with Singulair. My patient information guide shows that and the Dr. has confirmed that. It is a side affect I am willing to deal with so long as my asthma is controlled. I have cough asthma and I have tried just about every drug combo/type in the past 2.5 years with no real relief. So far, I am getting slowly better on the singuliar with Flovent 110 2puffs 2x and albuterol as needed. At this point I just want to know what works to relieve the headaches and if they will taper off as I adjust to the Singulair over time. From what I have been told, the answer so far seems to be yes. Lynn, When you have to start taking meds for the side-effects, there is cause to be concerned. What the headaches are from?? I tried Singulair and am considering it again (I still have 1/2 bottle left), but I did have flare-up while I was on it and went back to Accolate. I’m not noticing any side effects, otherwise I’d think twice about continuing. Better report that to the doc too. Since I started Singulair 10mg 1x tablets 2 weeks ago, I get headaches

The Prescribing Information shows that headaches with Singular are not significently greater than with placebo. See: www.singulair.com http://www.merck.com/!!vmIKI0y_2vmIKL0ZbE/product/usa/singulair/presc… Adverse Effects "Headache 18.4% with Singulair, 18.1% with placebo" So its rather unlikely that Singulair is the cause of your headaches. However you could try stopping Singulair for a few days to see, with your doctor’s permission. Headache is a known side effects of albuterol. Hmmmm. Good question. I thought Ventolin and Proventil (and generic albuterol/salbutamol) MDI’s were equivalent. But you are correct, the site says not only are the MDI’s not equivalent but other forms of albuterol (powder, pills) are not equivalent. However, Ventolin is made by Glaxo of England, and in the US is also supplied as Proventil by Schering under license from Glaxo. I doubt that the differences are significent, and I think if anything that Ventolin is the more reliable product. I think I did see a post that said Schering uses a different pressure in their propellant, but its interesting the other versions of albuterol are also not equivalent; maybe absorbed at a different rate. Ventolin and Proventil both have the same amount of albuterol, 90 mcg/puff at the mouthpiece [100 mcg/puff at nozzle]. My HMO provides generic albuterol MDI’s, Dey. boy, I thought I was going crazy expirencing this! like you, nothing seems to work, and my asthma doc is convinced there’s "no side-effects" with singulair…maybe it’s finally time to get a second (and third if necessary ) opinion! Judge me all you want, just keep the verdict to yourself

I had tried Accolate without much luck. I am now on Singulair. My doctor’s hope was not to replace the Flovent, but to reduce it. I used to trigger very quickly when I hit the top numbers of my yellow zone, but now I seem not to trigger so quick. This its self is a big help.

Hi, Accolate is a good preventive medication. There are some report that it can help some of the symptoms of allergic rhintis. I use it to some of my pateints older than 12 years old. For those with problems during or after exercise I use Serevent which lasts up to 12 hours. I am using Singulair, another leukotriene receptor antagonist to some of patients as young as 6 years old. It did appear to be doing wonders for my eczema, though my season for this is now over so it is hard to draw conclusions. I notice, however, that my symptoms were resurfacing when one of my main triggers rolled into town — high humidity for several weeks. The humidity having tapered down, though, I have even been able to get in some fairly intensive short-term biking runs with no problems (using salbutamol as a preventative prior to exercise) and I feel great afterward, my lungs actually working for once.
You should discuss an ‘asthma action plan’ with your doctor. This is a written set of instructions (tailored for _you_) that instructs when to increase or decrease your medications. I’d be interesting in hearing how others are doing with this and related medications. I’m particularly interested in the one which can be taken WITH food, as even two pills a day outside of meals can be a pain with my crazy schedules.
You seem to be one of the lucky people. According to the manufactuer about 75% of the people who try Accolate benefit – while antecodal reports in the newsgroup indicates that for 50% of the people it is the greatest stuff invented, but it has no effect (of any sort) on the other 50%. There is a new medication that has been released in the US called ‘Singulair’. It works in a manner similar to Accolate but only needs to be taken once a day and you do not have to worry about meals.

I started Singulair (montelukast sodium) about two months ago. It’s a once-a-day medication, which I take at night. I have been feeling much better, and have not used MaxAir for weeks (I still use Serevent twice a day). It doesn’t seem to have improved my breath meter results, but people tell me that I sound and look better. A number of friends and acquaintances have complained about their allergies recently, but I haven’t noticed anything. Also, I sing in a chorus, and I am now able to sing in a higher range with more volume than previously.

I’ve got what I would call mild but persistent athsma. My athsma has been worsening for the past five years, I typically have normal peak flow readings (when tested), and I’ve had a few real attacks. My known triggers in quasi order of severity are: cold/dry air(!!!), humid air(!!!), grass pollen(!), tree pollen(!), mould, mildew, cats, wet dogs (not dust mites). I suspect I have some very minor food allergies, but haven’t tested for them. I also live with three cats, to my allergists chagrin. I also have a mild case of eczema which flares up during the dry air conditions of the Canadian winter; I have developed very dry/cracking skin on my hands during the winter. Having spent a few years with only a rescue inhaler (Salbutamol/Ventilin), I started taking a low inhaled steroid dose (100-200 mcg/day) last fall due to worsening symptoms. Now that Accolate is finally available in Canada, I have started using it as a replacement for the steroids that I never wanted (6 weeks now). I have the prescription from the Dr. and now that my son’s full size sample is fast running out, no pharmacy has it yet, but they have all attended the seminar for learning how to dispense it and provide instructions to the patient. I live in Canada and started using Pulmicort 5 years ago. It has truly changed the way I live with my asthma. I have had asthma since 1976 and had tried almost everything on the market. I started using Pulmicort and Rhinocort (for sinusitus and nasal polyps) and I feel like a new person. I hope the FDA will approve this terrific product for your use in the U.S. (No, I don’t work for Astra) Best of luck to you in the new year. Your northern neighbor.

It’s pretty similar to Prozac in many respects but for some people the *initial* side-effects seem to be a bit more dramatic. It has a much shorter half-life than Prozac which means that a week after you stop taking it, the drug will leave your body – Prozac can stay around for much longer. There isn’t too much difference between the 2 drugs. Gareth.

Paxil helped me a lot more than Prozac ever did. The only bad problem I have ever had with Paxil is when I get a hair up my ass and decide not to take my medicine any more and tried to wean off of it. It is definitely a drug that has to be weaned from slowly. I’ve gotten very dizzy and had these shock like feelings going through my arms if I don’t take it. Ida Smurf <Smurf…@btinternet.com

I know I’m starting to sound like a walking advertisement for Paxil, but it’s helped me a lot. I’ve never been on Prozac, so I can’t compare the two. I did have some side effects, but they faded pretty quickly. Can I ask why you’re switching from Prozac to Paxil, and what specifically is worrying you about it? Serzone was the last anti-depressant I took last January through July. I have taken zolft, prozac, effexor, and trazodone when it comes to anti-depressants. Of all of them Effexor has worked the best. I was also on Serzone for several months and found myself tearful most of the time,almost suicidal. I talked with my Md. and was changed to Celexa. I have found myself much better. I posted this before http://www.ozemail.com.au/~drobb/bms/pi.htm for info on Serzone. It has some interesting things to say. I use Valium when I need it and passed on the offers of Zoloft, Xanax, Prozac, and the like. I was already aware, from first hand observation of those taking them, of what they are capable of. I will stay with what I have, thank you. It may not be popular but it seems to be a whole lot safer if you don’t need the effects that the others bring. It may not be the best solution but it is one that works for me to take the edge off when needed. The best part is that I was allowed to use the minimum dosage and when I need it. The hard part is trying to get through those times when you may want to take it but you know the feelings will pass faster than the pill can take effect so you just tough it out. If you can do that, you have half the battle won. Good Luck.

If you are hypo-thyroid and feeling suicidal 1) make sure your thyroid hormone level is adequate 2) get switched onto a T4/T3 combo And just because a TSH test was done and the numbers are in "the normal range" does NOT mean you are not hypo- there are problems with how the TSH normal range is calibrated, most MDs are not aware of the issues and questions about this. They are taught that the TSH test is definite, accurate and extremely sensitive — sensitive it is, but the accuracy depends upon how it is interpreted (not blindly) and it can never be definite — because there are forms of thyroid disease where TSH numbers become meaningless. T3 is one of the best anti-depressants in the shinks arsenal, most MDs however ignore or don’t know this. Depression and suicidal feelings are a symptom of hypo-, you may be undermedicated for hypo- or mis-diagnosed as NOT hypo- when you are. I have been on Serzone a month and a half. I am afraid to be alone any time at all and I have a lot of suicidal thoughts…how can I safely get off and what other anti-depressant could I mix with it and start taking immediately that would be safe? I also take synthroid and ativan. Before you buy.
Karen, The only idvise i can give, see your dokter. Efexor helped me get over the suicadal thoughts. taking now 300 mg, two times 150 mg. Kevin

You know, suicidal thoughts occur to a lot of people a lot of the time. There are days when it is a thought and something distracts you and you go about your business until the next time. Depression doesn’t help things either but staying busy and having an active social life is a tremendous help. Most of us here have had bouts with depression ( some severe and some accompanying PTSS ). My heart goes out to you. Please feel free to write if the need arises. I have found a link to the product information on Serzone. It may help you ( or even the others here to help ) you understand better how it works and if you should discuss it at length with your doctor ( my personal opinion from experience is to talk to them at length about it ). The link is http://www.ozemail.com.au/~drobb/bms/pi.htm and it goes into some depth so assistance from someone like Ted or one of the others that are more medically or chemically knowledgeable may be needed to understand it better and maybe explain it all to you in lay terms ( Lord knows I don’t understand it all ). If the thoughts of suicide become too much I suggest you get a hold of a friend and have them take you to the hospital for observation until it passes. Life is too short and precious to waste on a disease that is treatable. The hospital cannot keep you if you wish to leave so you shouldn’t worry about that, but, having caring people available with the knowledge to help will save you from doing anything rash. It would also be of benefit if a different type of medication needed in it’s place. From all that I can gather on Ativan, it is in the same family as Valium and has potential for dependence. Personally, I take Valium in as small a dose as possible when needed. I wouldn’t, however, discontinue use of your medication under any circumstance unless it is under observation and approval by a health professional. ( this doesn’t mean it has to be in a hospital but your doctor does need to know if you are discontiuing use as potential for withdrawl symptoms can cause complications ). Yes, you have a couple of issues that need to be resolved. I found that as the level of thyroid medication was increased I needed less and less of the Valium for the anxiety attack ( of course mine are PTSS related ). I’ve had to learn that they will pass in time and that I will be okay ( I can feel when they are coming on so I take a small dose of Valium at the onset ). Even without the Valium, I know it will pass but I try to stay active during the episodes ( it seems to help ). After learning to cope, the episodes tend to diminish in intensity when they do happen and so they happen less frequently than they used to. You may find you have similar result or find something else that works for you. In either case you need to talk to your doctor about it and discuss your options. Always remember you have people you can talk to and if nothing else, I monitor this group and my own mail regularly. Feel free to write if you need. You are not alone, even when you are by yourself. All you need to do is let someone know.

A month and a half is just enough time to get onto the first rung, re: dose. Since his dose is not working well enough, ask your doctor about another step up. It is a long process to find the right med and the right dose… but it sounds like it may be too early to give up on this one yet. If you are having suicidal thoughts, the important thing is to NOT be alone, while your meds are adjusted and stabilize (which can take many weeks). Don’t be discouraged…. but also, don’t be alone until you are at a dose that works well enough. I was on serzone for 8 months over which i went from 50 mg/day up to 600mg/day. I liked serzone re: lack of adverse side effects, but it just wasn’t enough for me. It was thoughts (and detailed plans) of suicide at the 600mg/day level that got me to change (i.e. give up). The difficult part in any change is the interface, the drop out from one med, the drop in of the other. If you reach a point where you need to change meds, do not trust yourself being alone during this period. Ask a close friend to ride with you. Or if all else fails, the hospital is there to ensure that you are protected during these transitions. There is no shame. The right medication will help, and you will be glad, and safe, and free from the suicidal thoughts…. once you find it. Again, I think it may be too early for you to give up on the serzone yet… try higher doses with your pdoc (what dose are you on now?). For many people serzone is very effective, and it’s the one they stay with…. once the find the right dose. best wishes, Han

Hi Jim, So it’s Saturday morning and I have a talk therapy appointment at 11. Went to the hospital yesterday to see my psychiatrist, and toughed out the (very) brief meeting with him, not wanting to appear too needy. I’ve been feeling very very crazy & suicidal all week and on Wednesday we upped my Effexor dose to 300mg per day.
Are you still taking Depakote? I had prepared a long logical argument with graphs to convince him that the antidepressant side effects were actually causing more depression than they were treating, but he wanted to try more instead of less (I suggested sliding down from 225mg to 150mg). OK. One of my past bad habits has been becoming so upset & mistrustful of my mental health caregivers that I just bail out. I’m trying to trust this young guy (I’ve been taking SSRIs longer than he’s prescribing them). I think this 300mg dose is starting to undercut the good effects of the Depakote (1250mg).
It may be….I guess only time will tell. Has he considered the possibility of adding a 2nd adjunct MS to the Depakote? But then I had to deal with the billing process and the Medicaid office and the damn back & forth (I’m broke & have no money so the clinic will offer me a sliding scale, but because I need meds I applied for Medicaid, but because I applied for Medicaid the clinic won’t offer me a sliding scale and I won’t find out for 2 weeks whether I’ve been approved for Medicaid und so weiter). It was real bad, very Stanislaw Lem.
The bureaucratic run around is so frustrating….and IMO unnecessary. This is a stressor you definitely don’t need (nor does anyone for that matter)!! I feel like I’m failing, like I’ve failed, like there is nothing ahead of me but decline. I’ve applied for SS Disability but have been denied once and the case is up for review. I haven’t paid rent yet this month, and although my sister has offered financial support, I can’t bring myself to actually avail myself of her help.

When I first checked out this newsgroup, I saw a very long and angry rant about (text in quotes roughly from memory) "whiners who expect a handout…should get up off their butts and go out and work for a living…" or something to that effect.
Some of us can, and some cannot…through no fault of our own. I have an internal Republican who also says that kind of stuff to me all day long. He’s also an adamant Social Darwinist who tells me that if I don’t have money I’m worthless and that I should just get off the planet.
You are NOT worthless Jim. Just try to let that thought go!!! I dunno. My history is pretty checkered, but I did manage to keep various jobs for about 27 years. My last job was a vice-presidency. I would really like to get back to work sometime soon, but I feel so sick all the time now…this has been the worst 5 years of my life. SSD would provide a little cushion so I could eat better, not worry so much about turning into one of the myriad homeless, and get back on my feet and put myself back into the world.
I do hope you recieve it soon. Most of the time I think suicide is my only option. I’m so tired of all the pain I live with and I can’t imagine things ever getting any better. Maybe I am just a drain on the resources of others and getting out of everyone’s way would be for the best. If only I could just stop my heart….
I share similar sentiments Jim. I’m so tired of the "med go round." It is neverending. I am filled with hoplessness too <sigh. Mostly what I’m doing is just enduring. Plodding through another miserable day. No suicide, just torture. I have no other tools…. Sorry for the long rant.
