LONG = Just Out of Hospital — what now?

I am 39, diagnosed with asthma at 34 — although in retrospect, I suspect I’ve had it much longer (but I never complained to the doc. about my shortness of breath because I’m overweight and "knew" that he’d just tell me to loose weight). When I was first diagnosed at 34 I had quit smoking for about a year — then I started again about 1 yr after diagnosis (we won’t discuss that stupid decision). Anyways, I’ve always viewed my asthma as a very "minor" issue in my life. I took my steriodal inhalers — occasionally had to double dose in the AM — but generally I did pretty good. I quit smoking on August 11 of this year. On August 19 I woke in the morning and after ventolin managed a measely 150 on my Peak Flow (when my asthma is sort of in abeyance I blow around 400). Went to see the Dr. and was put on Prednisone — so I was taking Pred. 40 mg/day, Becloforte 2XDay, 4 puffs, plus ventolin. Things seemed to get better — off the pred. on August 25. On Sept. 1 I made a visit to emergency for a nebulizer and was put back on pred. On the 2nd, I ended up in hospital with asthma problems but with the additional diagnosis of pneumonia in my right lung. I was released today. SO…. still on Pred. with all the inherent nasty side-effects there, eurethromyecin (antibiotic), ventolin, becloforte (soon to be flovent)and alledgedly on the mend. However, I DON’T FEEL ON THE MEND. I feel much better than I did, I can get air into my passages, sort of, my peak flow is hovering between 220-260 (before and after ventolin), while after the nebs in the hospital I was getting about 310. I’m not wheezing. But, my lungs are tight, it hurts to inhale deeply, like I should, and I have so far resisted "double dosing" my ventolin, but the temptation is coming closer since I know that if I take my dose, wait 5 min. and take another, I can actually breath. Having now discovered "what" the problem is with asthma — can anyone tell me what to expect from here — what should I do, not do, see, take, not take….I’ve read the site that John Rhoades is linked too and the FAQ — I guess what I’m looking for now is some quick tips for the fast cure. I’m back to see a Doc. on Sept. 11 — this is all complicated by the fact that my Dr. has left town and I was in the process of getting a new one with this "crap" all happened. So….tell me — What should I be doing, saying etc. before and to the Dr.? All words of advice appreciated.
Under current US Guidelines, you should also be taking a long-term, bronchodilator; either salmeterol (Serevent) inhaler [2 puffs x 2] or theophylline SR [TheoDur] tablets. Ex-smokers sometimes benefit from using the Atrovent inhaler. A new inhaler, Combivent, combines Atrovent and Ventolin in one inhaler, for those who need both. It would be advisable to have lung function tests in a pulmonary test lab after you recover, to determine how much of your problem is asthma and how much is chronic bronchitis or emphysema. The later 2 conditions tend to be caused by smoking. You may benefit from programs in smoking cessation, diet, and exercise. Ellis

I am not a doctor but I have had similar problems with breathing during and after prednisone. Prednisone tends to bloat you just as eating a large meal can. Your stomach can push your lungs up and it becomes very hard to breathe. I would suggest to you, just as my dr. did to me, eat very small meals about 5-6 times a day. Your stomach will never be as full. Pneumonia can also damage your lungs somewhat. I can even be responsible for the onset of emphysema, especially in us astmatics. I have asthma and emphysema. WK, Now that you’re getting some relief with Xanax, are you going to try getting back into the work world? Considering how long you’ve had to stay away, you might want to check with your local gov’t., your doctor, whoever, to see if there is "dept." set up to help people with mental disabilities re-enter the work force. I’d like to say that potential employers will probably just gloss over your 2 year absence, but that’s unlikely. Sad to say, but there’s still tons of ignorance and some stigma attached to having a disorder. Anyway, getting back to some sort of go between organization, if it exists, (and I know it does in some states in the U.S.), such a dept. can smooth the waters some in locating employers who’re interested in what you can do and will have been properly briefed that you have an anxiety disorder, but are being treated for it. Something else to think about, and you may already be doing this, is that for an interview or any particularly stressful event, I’ve found that taking an extra dose or two of Xanax all at once just before the ordeal can make it a lot less stressful. I had to experiment some to find the right amount and you may too (not enuf vs. too much). Good luck. I am sick and tired about all these posts trying to scare people away from trying xanax. As someone who has suffered from bipolar disorder(type 2) for 15 years, tried all the antidepressants, now on a cocktail of meds, including xanax and paxil, my experience is that PAXIL has many more adverse effects versus the marginal benefit of some anxiety relief. I am a college graduate in electrical engineering from a top 5 engineering college, and now have not worked for over two years mostly because of anxiety and some depression yearly. After working for 11 years struggling with depression and anxiety, I now have a hard time stepping outside my home. I have considered suicide 1000’s of times over the last 15 years, actually researched and made the plans and was about to end it, and then I started on the Paxil…….Yeah, my suicidal obsessions went away, but now I was content to veg out all day watching TY or on the Net, my desire to GET A JOB disappeared. Go research web sites and you will see how many people have problems with SSRI’s, yeah they are alive and taking that $3 pill two times a day or many $3 pills for me, but now I am NUMB and just there. Prior to Paxil I use to be a emotional person, felt the pain of others and would cry when touched by something, but now I am a Paxil-zombie, yeah some anxiety relief but a LOT OF ME is also gone. So all those xanax phobics, please allow people to fail with their options without scaring them. What are people’s choices when it comes to anxiety or panic, yes paxil, yes benzo’s, yes MAIO’s, yes CBT(Cognitive Behavior Therapy). My experience is PAXIL SUCKS and THE WITHDRAWAL IS HORRIBLE BASED ON MUCH FEEDBACK I HAVE READ ON THE NET, MAIO’S like parnate have some studies that back them up but the food restrictions are VERY DIFFICULT(no cheese, wine, beer, chocolate, many OTC meds, etc.), CBT might work for some but the feedback I have heard is that CBT plus meds work the best and I HAVE DONE THE THERAPY ROUTE and it was a waste of money. As one doctor told me, people who suffer from PANIC and think they are dieing have never been told that "your not going to die, it is all in your head"…Well that was my last session with him. Just to repeat, this is MY EXPERIENCE and YOUR MILEAGE MAY VARY. Now I could continue to HIBERNATE IN MY HOUSE, NOT WORK, RUN OUT OF MONEY AND THEN BE HOMELESS…..Or I can be proactive and continue to experiment and try to find some solution that will allow me to work and have a life. People need to do some research and FIND OUT THE REAL TRUTH, not what BIG PHARMA and BIG BUSINESS OWNED MEDIA OUTLETS want us to believe. Not to be cynical, but $3 no generic available paxil is much better for DRUG COMPANIES, then CHEAP GENERIC XANAX which might be more effective with less side-effects. Which posts? Anyway do you get pre, during and post anxiety or just anticipation and during anxiety? Cause I get all three and am curious about your experience. I the amount I get of each one depends on the situation. Richard

Just as all the SSRI’s go generic, we will hear about ALL THE NEGATIVES and out will come the new batch of 8 TO 12 WEEK STUDIED ANTIDEPRESSANTS to start the new profit cycle. People with mental illness are just pawns, half ass useless meds are approved based on 8 week studies, NO REASON FOR A LONG TERM STUDY, we really don’t want to KNOW THE TRUTH, and as soon as the generics start rolling out, OUT COMES THE DIRTY LAUNDRY, not to HELP US, NO, ONLY SO THAT WE GO ON THE LATEST $5 PILL THAT HAS A 80% SUCCESS RATE AND EVEN LESS SIDE EFFECTS. Hell as long as everyone is making money, who cares about THE CRAZIES that take the product, we just need them alive and taking their meds. Now I forget, was I taking about the cigarette industry or drug companies, HELL WHAT IS THE DIFFERENCE. I remember day 5 thru 10 to be the worst. day 15 (sweating profusely, brain shocks,couldn’t eat, or drink coffee, drank huge amounts of water) , it was had already tapered down. Day 30 I had brain shocks maybe once every 2-3 days, but other than that I was pretty normal Week 8 I was completely back to normal. BTW, if you sample a fair amount of posts, you’ll find that they are all in their first 2 weeks of withdrawal. After that not many people post anymore. That is an encouraging sign.

Unfortunately Effexor is associated with withdrawal problems that can be just as bad as Xanax withdrawal. A way to get around it is switch to Prozac and then taper off of Prozac which is much easier to stop. Philip

The worst withdrawal I ever had was from quitting good old fashioned coffee cold turkey. I was in my 20’s and lived in a very rural area, was doing a pot of coffee a day. Big snowstorm showed up, out of coffee, no way to get to town to get my fix. Horrible headaches for 3 days, stomach cramps etc. I gave up coffee for awhile after that, went back to it, later decided to cut down, tapered slowly, no withdrawal. Now I do 1 mug of black coffee each morning, forgot it one morning and didn’t notice. I tapered xanax last summer, cut down by .25 mg every 3 days, no withdrawal, stopped it for nearly a month, then some serious shit happened in my life, a family member phoned screaming at me at 2 am and I ran for the xanax bottle. Xanax not only keeps me sane, it keeps some people who have to be around me sane too. I don’t recommend quitting cold turkey for any substance. I developed adverse side effects. The worrying one was high blood pressure so I had to switch ADs. It was also pooping out as an antidepressant. I had climbed to 300 mg over the years but my depression was not controlled by the end. Meryl

Thanks. Sorry about the repost that appears as gibberish. I had previously used an anonymous posting service but thought it did not work since I got no response for hours. I also did not know the posting service posted everything in html. Yes, that is correct. when off zoloft I get impulsive and aggresive. I will agree the Zoloft is helping but I do not want to take it forever. It has many side effects as you probably know. I actually liked the feeling of being ‘high’ when being off the zoloft. I had forgotten what it was like. I only restarted to take it to see if I went back to my previous socially more likeable state. I also started back to calm down a bit and make sure I did not do or say anything impulsive that might cause problems at work. My goal is to find out what is causing the non-zoloft feelings and behaviors. I am beginning to think definetely a physiological basis but sometimes I wonder if it is some sort of personality problem. This is the first medicine I have taken my whole life that has made a difference but I would like to see if there is another way to get cured. If you find that you still get episodes of the feelings you don’t want, even while taking the ‘Zoloft’, then you need to tell your doctor so that other treatments can be considered.

There can be lots of things like that. I think twwells does put a bit of a delay in getting messages out, but others in ASD do use it. I think your news-reader program (Outlook Express?) may be set to use ‘Rich text’ or HTML – if you try twwells again with your software set to use ‘plain text’ things might work better, and I’m sure it’s more convenient than posting via Google. Talking therapy helps many people work out stuff like that. Zoloft (sertraline) is an anti-depressant, so if it helps then Depression might well be at least part of the problem. There are many other anti-depressants if there are side-effects of sertraline that you can’t accept – a doctor would be able to advise. I am beginning to think definetely a physiological basis but sometimes I wonder if it is some sort of personality problem.
Depression /is/ physical – chemical neuro-transmitters get out of balance, and anti-depressants work by doing various things to adjust the balance. How things get out of balance in the first place, is not yet worked out; there may be several causes. As far as I know, personality disorders are best diagnosed by a psychiatrist or psychologist; your doctor could probably tell you more about that and possibly arrange a referral. This is the first medicine I have taken my whole life that has made a difference but I would like to see if there is another way to get cured. Maybe its a sugar problem, hormonal problem, etc and the zoloft just softens the blow of the physiological imbalance. bg

I started out with the diagnosis of major depression, dysthymia and PTSD several years ago. It took over a year to find an AD that would help at all. I tried Wellbutrin and Serzone and neither one helped my depression. When I got on Prozac I finally felt some relief from the depression but only part of the time. I would feel better for awhile and then crash very hard. Finally last October my pdoc suggested I was bipolar II and put me on Neurontin. I am still undecided about it. There are days I think it helps a lot but not always. My pdoc says that is because I am not at a high enough dosage so I am raising it slowly. So I guess I still don’t know if I have the right combo or not. All I know is that it sure is a pain trying to find the right one. Bonnie

The Remeron was the third. Was given Buspar, didn’t work out. Took Triazolam for sleep, that worked well. I now rarely use the Triazolam and take 45 MG Remeron daily. I have been at this for about 18 months and what a difference! The best advice, if you find something good, stay with it! Never happened for me.

I was lucky – I stabilized pretty well fairly quickly on Lithium and Effexor. With my son, on the other hand, it’s been a two and a half year ride I wouldn’t want to repeat (but unfortunately isn’t over). He started on Depakote and Risperdal in his first hospital stay. The next hospitalization added Lithium, which I saw a dramatic change for the better on. Then he gained 70+ pounds on the Risperdal, so we moved him to Geodon last summer. In October/November he began having acute anxiety symptoms, so a minimal dose of Risperdal was added to the Geodon. The anxiety isn’t really falling, and he’s had yet another hospital stay, so his pdoc and i are considering an anti-anxiety med, but we want to wait and see how much of the anxiety was fueled by the holidays before adding another med to his cocktail. Sigh. All this and weekly therapy outside of school, weekly therapy inside of school, and putting him into a special program at school to remove him from the mainstream (he has a tremendous amount of school phobia). Is it any wonder I’m a bit wacko these days?

combo? Prior drugs I’ve taken were lithium (I was a zombie) , sinequan (great sleeps, but not much else), anafranil (excellent for my OCD, but was really shaky). I just started Depakote on Wednesday, so I’m not sure, yet. Zoloft has worked great for my depression, though. How many attempts did it take before you found the most effective med combo?
Neurontin, Tegretol, Topamax, Klonopin, Ativan, BuSpar, Trazodone – in various combinations. I had severe allergic reactions to both Neurontin and Tegretol; Trazodone did nothing; BuSpar helped my anxiety, but what I had really needed was a mood stabilizer; Ativan and Klonopin helped my anxiety, also, but I prefer to use benzos as little as possible; Topamax didn’t really help my mood, and the change in taste and tingling sensations in my hands and feet annoyed me. I’m considering adding Topamax to my current med combo, because of it’s unique side effects profile(i.e. promoting weight loss). I’m going to talk to my pdoc in a couple weeks, about adding either that, or Meridia. So far, Risperdal and Depakote have not caused weight gain, but I’m seriously overweight as it is, and the thought of more weight gain frightens me. Any suggestions(Oh, just for the record, I’m 16 years old)? -Jonathan

I started out with Lithium and Tegretol. Within six months I had developed psoraisis from head to toe. My skin turned very gray. I had severe tremors. The doctor took me off the above two drugs and tried many others over the next four years. I can’t remember how many different ones but the list came out to three computer pages long. We finally found the right drugs: Prozac and Xanax. And a year and a half ago I started Depakote. This year we added Vicadan (sp?) for migraine headaches. I avoid the Xanax and Vicadan as much as possible because they are so addictive. I’m usually pretty sick before I take either one. I still see and hear things that are not there. But most of the time I can handle the voices and visions. I still fight the suicide urges every moment of my life. If I was crooked I could con people and start a new religion. But I’m not made that way. And I would have to account for my behavior in the next life. No thanks. Enough of my senseless ramblings. Richard

I was lucky – I stabilized pretty well fairly quickly on Lithium and Effexor. With my son, on the other hand, it’s been a two and a half year ride I wouldn’t want to repeat (but unfortunately isn’t over). He started on Depakote and Risperdal in his first hospital stay. The next hospitalization added Lithium, which I saw a dramatic change for the better on. Then he gained 70+ pounds on the Risperdal, so we moved him to Geodon last summer. In October/November he began having acute anxiety symptoms, so a minimal dose of Risperdal was added to the Geodon. The anxiety isn’t really falling, and he’s had yet another hospital stay, so his pdoc and i are considering an anti-anxiety med, but we want to wait and see how much of the anxiety was fueled by the holidays before adding another med to his cocktail. Sigh. All this and weekly therapy outside of school, weekly therapy inside of school, and putting him into a special program at school to remove him from the mainstream (he has a tremendous amount of school phobia). Is it any wonder I’m a bit wacko these days?

I started out with the diagnosis of major depression, dysthymia and PTSD several years ago. It took over a year to find an AD that would help at all. I tried Wellbutrin and Serzone and neither one helped my depression. When I got on Prozac I finally felt some relief from the depression but only part of the time. I would feel better for awhile and then crash very hard. Finally last October my pdoc suggested I was bipolar II and put me on Neurontin. I am still undecided about it. There are days I think it helps a lot but not always. My pdoc says that is because I am not at a high enough dosage so I am raising it slowly. So I guess I still don’t know if I have the right combo or not. All I know is that it sure is a pain trying to find the right one. Bonnie

Mucking around with hormones is not a good idea. Also true HGH is a very expensive injectible presctiption-only substance, not something you can buy retail. It sounds like you are a short 22-year old male who wants to be taller. Unfortunately, genetics and nutritional status during childhood and adolescence have almost complete control over height. One of the 18yo boys was having some joint pain recently, and since they’ve been slow growing (and are still a bit shorter than their father) we thought maybe it was a late growth spurt causing something like Osgood Schlatter’s Disease. Nope. Hi Manuel- I personally grew 2 inches at age 30 as a result of heavy therapy which cleared out some difficult childhood trauma. HGH (whether injectable, releaser or homeopathic) has NOT had very good results with people like yourself. But if for years you have felt yourself to be small, short, unhappy about your height, etc; there is a possibility (my GUESS would be about 3%-10% chance) that HGH might do something for your height partly because of psychological reasons. Bad Mornings
Question:
The worst time of day for my crying and upset begin shortly after waking and continues through shaving and breakfast, and lasts until about noon, often very severe crying. Somewhere in mid-day I usually stop crying and can function. Occasionally the crying will spill over into the afternoon, but not normally. This general pattern has been present for some months and with various medications. My medications at present are: Welbutrin 200 mg in morning Zyprexa 10 mg at night Nortriptyline 75 mg at night I also had 14 electro-shock treatments in June/July. My question for this post is to ask whether others have a ‘bad morning’ experience. Thank you. John

Thanks jai for your suggestions. Yes my pdoc is well aware of the continued failure of various medications over the past several months; I have used a variety of medications both singly and in combination over the past year: prozac, zoloft, effexor, remeron, etc. none of which brought me out of the pit. I don’t know what Nortriptyline is for. My worst times for anxiety are in the mornings. I don’t want to wake up some days because that means I have a whole day to get through.This sometimes actually leads to me waking up earlier and earlier in anticipation of the fear and anxiety. It’s worse in the winter. It sounds like perhaps this med combo isn’t working for you. Have you been on it very long? There is no magic pill or med of course, but some of us do find different meds/combos to be of help. Have you told your pdoc about this? Take care. Mine’s usually late afternoon. Mornings are reserved for that wonderful "severe fatigue". So tired I can hardly focus. — Amelia

I’m sorry you’re having a hard time, John. But at least you’ve got lots of company in this dark hole. When just 1 of us gets a few inches up out of that hole we need to yell back down and make sure the others know that we see light. Deal? — Amelia

what and when are you eating?
Usually breakfast around 9 or 10 a.m., I simply ‘can’t’ get myself up earlier — I used to but not this year. Lunch around 1 or 2 p.m. Dinner at about 7 p.m. I consider the food I eat to be plain ordinary meals. if you are not eating before these times you could be experiencing low blood sugar which could cause/contribute to depression.
At one time in my life I did have a problem with ’sugar dips’ [ hypoglycemia] and the resulting feeling faint….what I have now doesn’t seem at all like the old sugar dip problem [which has been gone for several years]. since you experience these feelings even while taking meds, have you considered other factors?
Good point — my pdoc did have an MRI done of my head to rule out something physical there…she says she checked my thyroid, but I have no record of it, and plan to raise that question again next visit [which is next Tuesday]. Thanks for the input. John [age 63]

<< My question for this post is to ask whether others have a ‘bad morning’ experience. My worst anxiety also is in the morning. THis has been happeing for the last seven months of this depression/anxiety illness. I am not currently taking anti’s as I have tried them and they only made me worse. The anxiety and panic attacks are getting better and some of the detachment syptoms, however the depression is still very bad. I have been praying and trusting God for the rest of the healing with proper nutrition, excercise and rest. Yvonne

Dizzyness anyone?

Hello all, I started getting panic attacks about 6 weeks ago. I was diagnosed and given medication (Xanax and Zoloft). The intense attacks have disappeared, but I would say that I am dizzy nearly all of my waking hours and I can often go into some sort of trance and convince myself that everything is unreal. Does anyone have advise/experience with dizzyness? I can understand it coming during an attack, but I am literally dizzy all day long! Help! Thanks, Greg

Greg, Sounds like you may have vertigo, in which case you should probably see a neurologist for testing. I, too, have been having problems with dizziness. One resource that helped me a great deal was the Vestibular Disorders Association (VEDA). I’ve forgotten what the internet address is, but it should be easy to find if you do a Net search.

I wouldn’t go straight for the neurologist at this time, I would find a good ENT (ear nose and throat dr) The neuro. should be reserved for last, Since any good ENT can probally diagnose the problem. Greg, It may well be the Zoloft, which can cause dizziness (SSRIs can cause just about anything). You may want to talk to your doc about trying a different SSRI, Prozac would probably be the best choice, IMO. There’s lots of complaints (more than with other SSRIs) about Paxil’s side effects (though some do very well on it), and Luvox is less effective than the other SSRIs for panic for quite a few (though, again, many do well on it). My therapist thinks there’s something I’m not telling her, and my PDoc keeps upping my Zoloft dose. He says that any other meds are a ’shot in the dark’ as to their effectiveness with me, and since Zoloft worked for me once (before I went off it-not recommended!), it should again, but I’m at double the dose now w/ no relief and the side effects are increasing. My wife is getting frustrated, since I’m such a burden when I crash, and it takes me *so* long to get back to ‘normal’. She’s a rock, but after an episode, she tends to get a bit more upset with me since it builds up. At least I had one good realization while I was down: I felt that it *would* pass at some point. That’s a first. So score one for my sense of self-preservation. Thanks to all the asd’ers and grinchers that supported me last week. I hope I didn’t bring you down. Mark, *Any* med is a shot in the dark. The fact that Zoloft worked once, doesn’t mean it will work forever for you. Since the side effects seem to be increasing and the Zoloft isn’t working, maybe it’s time to try something else. You’re paying the pdoc for help, insist that you get it. chuck
