It is debatable which is better. Pulmicort has less systemic exposure due to rapid metabolism in the liver. Also the delivery device eliminates the need for a spacer. Personally, I use Flovent more because I think my patients are more used to the MDI and I try to get them to use the spacer for the other MDI’s as well. There doesn’t have to be an insidious reason for the recommendation to switch. If it is due to formulary reasons I think the doctor should be excused for not fighting a switch to a drug that is as good, if not better. — CBI, M.D. Please note: It is impossible to accurately diagnose medical problems without seeing the patient and reviewing the entire history. I used Maxair and Asthmacort successfully for 12 years. Last year at this time, my asthma started causing me trouble. And this has gotten steadily worse over the year. Question 1: Do these drugs become ineffective after long-term use?
Typically, no. However if your asthma changes then they may no longer be adequate. Question 2: If Asthmacort is no longer effective for me, will Pulmicort do any good?
I stopped using Azmacort a long time ago. I have been on Pulmicort for about a year now and have had great success. "Keep looking below surface appearances. Don’t shrink from doing so (just) because you might not like what you find." General Colin Powell

My sleep doc appt.
Question:
Finally saw a certfied sleep doctor last Monday. After dealing with my family doc and an ENT that wants to do surgery, it was refreshing to meet with a real sleep specialist. He went over my history with me, reviewed my 2 1/2 year old sleep study and gave me a thorough exam. He told me under no circumstance would he recommend any kind of surgery for me. He feels that the CPAP will do the job for me, but to be absolutely sure, he has scheduled another sleep study. This will be at a different sleep center than my previous study, one that he normally works with. One of the reasons for another study is that besides the obstructive events due to the upper airway resistance syndrome, he also noticed abnormalities with my REM and non-REM sleep. Apparently I never get into the restful sleep, obviously making daytime exhaustion worse. Since I’ve been on Effexor for depression for the last few years, he wants to make sure that it’s not the cause of the REM sleep problems. Therefore I’m gradually weaning off the Effexor. I need to be completely off of it for 2 full weeks or more prior to the test. I like the fact that he picked up on this from reading my previous study and wants to eliminate it as a possibility. If first impressions mean anything, I got a real feeling of confidence with this guy. The sleep study is scheduled for Dec. 12th. Didn’t want to wait this long, but it’s necessary so that I can get off Effexor first. Who knows, I might really get somewhere this time instead of being told there’s nothing really wrong. I’ll keep you updated with what happens. Denny

The problem, Denny, is that they convince you it’s just a couple more months every couple more months, until you’ve realized you’ve had 4 tests, 8 docs, 3 psychiatrists to top it off, and a pulmonary specialist who’s never read a single bit of research on Xyrem telling your psychiatrist (who knows better and tells you he just doesn’t want to conflict with his hospital collegue) not to treat you with something that is very likely to turn your life around. People who are tired have a life long condition, low patient outcomes by docs and patients alike, this leads patients being convinced they’re being helped by simply repeating the same tests, or getting a new diagnosis that meant the same thing as the one that replaced it 15 years ago. Treating you and validating your need for treatment are separate parts, and while I understand you need to wean off Effexor, your doc hasn’t told you you’ll need to wait a few weeks after that to have a PSG, wait 3 weeks for that to be analyzed, another appt. with your doc to follow-up on it (where all he does is order a titration study he could’ve done over the phone) and another few weeks to book that and then 7 weeks for you to E-Mail the doc to ask why you haven’t received your report or prescription yet, a lie that an E-Mail went out much earlier, and a week for the CPAP to be installed, then a few weeks for the correct mask to come replace the nasal mask, then several weeks to notice if the CPAP alone will do a darn thing. I’m at this last point, and crossing my fingers after a year of this nonsense.

Hi Denny- I’m sorry the other poster is having such a terrible time- but it certainly isn’t the usual. (Unfortunately it’s not rare either.) But to give you another experience, I went from my first call for an appointment to having my CPAP in about 2 months. (I know that’s exceptional in the opposite way.) Actually, since you need two weeks to get off the Effexor, you’re moving along in the process pretty quickly. It does seem that this guy is more knowledgeable and taking into account a much wider range of issues. Sleep docs have very specialized knowledge that others just don’t. Especially ENTs who SEEM like they would because they know about that part of the body and therefore people take their advice when they really don’t know SLEEP issues that stem from that part of the body. A perfect example of "A little knowledge can be dangerous". So anyway- enough preaching. Hope the rest goes as quickly as the beginning. Susan – Hide quoted text — Show quoted text -Ganishe wrote:

it’s great that your doc is concerned about REM/deep sleep issues…….so many times here we hear of people’s results saying "no stage 3+4 sleep" yet the doctors completely ignore it. If you read the clinical information on venlafaxine (Effexor), the two week period has to do also with trying to avoid withdrawal symptoms.

It’s also worth noting that poor sleep as a result of OSA can have a significant negative impact in general on metabolism, can result in irritability and general outlook on things. So, if your sleep problems can be improved or resolved, it could be a benefit all round for you. Definitely this exercise is worth doing…… .andy To email, substitute .nospam with .gl

Withdrawals from SSRIs
Question:
Hithere… for anyone who cares to know. I am doing remarkably well on day 2 at 25% of my regular 50 mg Zoloft dosage. I will remain at this dosage until after my menses have passed as PMS can be hellish. Happy as a Clam… PW
Hello. I have dealt with anxiety and secondary depression for about 15 years. I have been misdiagnosed more times than I have fingers. I am under the impression that diagnoses are merely a way for the
psychology/psychiatry – Hide quoted text — Show quoted text – industry to allocate medication. It has nothing to do with management. I have been on Prozac, Melaril, Paxil, Zoloft, Depakote, Risperdal and Xanax. Of all of them I found Zoloft to be the most helpful. Unfortunately, one of the side effects is weight gain. I have been on it, 2nd time around, for about a year and a half. I have learned of a new pseudo-natural supplement called L5 Hydroxytryptophan (5HTP). It’s a lot like Tryptophan in that it helps the body produce serotonin and melatonin. I lost my job about 6 months ago, as I go through them like kleenex tissues, and, subsequently lost my health insurance. I cannot afford to see my psychiatrist anymore. I have always been inclined toward natural healing techniques and really wanted to come off the Zoloft. I also wanted to lose the 30 pounds I gained in the last 9 months. I tried to come off it and I started feeling edgy on day 3. On day 4 I was as maniacal as I was at my absolute worst. I did some research to learn that there are indeed significant withdrawal symptoms associated with SSRIs not excluding permanent neurological damage and death. Of all 20 or so doctors I have seen in the last 15 years nobody ever mentioned this! I was appalled. Had I known this I may have looked deeper into alternatives. It’s like coming off heroin! I am on my 6th week of tapering off. I have had to cut my 40 hour week (at my new job w/o insurance) to 20. Fortunately I have a job where this is possible. I am consulting with a nutritionist on my diet and 5HTP. She has personal experience with the same type of disorder manifestation as myself except she was on Prozac prior to using 5HTP. What I would like to know is if anyone reading this has any experience coming off SSRIs and/or using 5HTP. I am using kava kava to help keep me calm during the day while lowering my dosage. I use a subligual form of melatonin at night to help me stay asleep, as I have a tendancy to wake up every hour. It’s working so far. I am down by 50% of my 50 mg dosage. To women out there: I do not recommend adjusting your dosage during PMS. Most of us have it worst during this time. I adjust my dosage about a week into my cycle – when I am at my best. I have had the best results this way. Also, it takes me about a week to adjust to the lowered dosage. I have been working with this for nearly 2 months now. I found that using 5HTP with the SSRI is not good. I was flooded with serotonin and nuts as ever. I’d love to hear other ppls’ experience coming off SSRIs and the use of natural "supplements". Thanks! Ms. PW
Response:
I suppose you know that kava is not recommended for use with any other psychoactive drug. Kava is known to potentiate (increase) the effects of other drugs e,g,, benzodiazepenes, alcohol and barbituates. I haven’t seen anything specific to SSRIs or other drugs that affect serotonin, just educated speculation that kava could have a negative impact on serotonin utilization. Another poster related a problem with anxiety rebound when taking kava, a report I’ve never seen before; I wonder if he/she was taking another drug at the time. From your last post, though, the combination seems to be working for you. I’d suggest, though, that you keep a close watch on your reactions and discontinue the kava as appropriate. I’d be interested in hearing how it goes for you. I gather that your intent is to withdraw from Zoloft and start on 5-HTP. May I suggest that you hold off on the 5-HTP and just stay on kava for awhile? If you have anxiety-driven depression, you may not need anything else, or perhaps something milder like an omega-3 supplement (skip St. John’s Wort – it eats kava as well as birth control pills, HIV medication, etc.) Anyway, take the time to evaluate yourself before you take on the possible risks associated with 5-HTP. (Actually, I’m considering taking 5-HTP in lieu of melatonin – still researching the issue of combining it with kava) One other note about kava: researchers may have found the reason why some users have suffered liver damage. It seems that some herbal manufacturers have been making their kava extracts from "peelings", the bark of the aboveground stems of the plant; which contain pipemethystine, a substance proved to be toxic to liver cultures. Peelings are a by-product of kava use in the South Pacific and are normally discarded as waste material; in 1998, though, 82% of the kava imported into the U.S. consisted of dried peelings. It would be reasonable for all kava users to research the product they use and determine whether any of the above ground portions of the plant (peelings, stem or leaves) are used; if so, change to another brand that only uses the underground portions (lateral roots and rootstalk). You don’t need hepititis or cirrohsis on top of your other problems. You may also end up with a more effective kava :}. Cheers, Figaro
Response:
I am reevaluating my need for 5-htp at all. Kava does seem to be working for me. I actually only need it on days 1-3 (about) after downgrading my Zoloft dose. After that, I seem to level out naturally, except for the use of sublingual melatonin. I think that working only 20 hours a week really halps too, though I can’t do this forever. I have forwarded your message to my nutritionist. I am particularly interested in what she has to say about the peelings. The brand of kava I use is Gaia Herbs. I’ve used other supplements they manufacture with positive results. I have heard of the anxiety rebound effect of kava though, as of yet, it’s not happened to me. It may be because I don’t use it all the time, only while transitioning to a lower dose ssri. My most annoying withdrawal symptoms now is nausea. It only lasts for a couple of days and it’s mild, but it makes eating difficult. The other side of that is that I eat less. Since I gained 30 pounds on Zoloft, I’m not complaining too loudly. I anticipate it stopping after I come off the ssri completely. I have lost 10 of the 30 pounds in the last 6 weeks. Every time I downgrade my dose I lose 3 pounds in the first 36 hours! Woo-Hoo! PW

Another poster related a problem with anxiety rebound when taking kava, a report I’ve never seen before; I wonder if he/she was taking another drug at the time.
That was me. The only other drug I was on was alcohol… and lot’s of it. I was self medicating for about 25 years. Over 15 months sober, one day at a time. Tono

Thanks for posting the info, Tono. I just try to collect info on kava whereever it may be!

LM, Thank you for your information. I am working with a nutritionist who has an MS. She has informaed me that the blood levels of 5HTP are a concern only for those very few with serious metabolic issues. I am quite healthy in that regard. She assures me that in her years of working with the CDC she participated in clinical studies on the use of 5htp and tryptophan and found that both amino acids are quite safe in healthy individuals. She has several patients who are taking up to 250 mg of 5htp daily and have for a year or better with no problems at all. In my opinion, the ssris are a much greater risk for me personally since there are no clinical studies on the effect ssris in ppl taking them over one year. The side effects of ssris have been devastating for me. For those with metabolic issues it should not be taken lightly though. Thank you. I will keep the group informed on my progress. PW
Thanks! Ms. PW please do not take 5ht without discussing it thoroughly with a doctor-it can cause elevated serum levels of serotonin which does not pass the blood brain barrier but can destroy the valves of your heart-tryptophan as a supplement will not do this because the metabolic process of converting tryptophan buffers the larger flooding of blood levels if you truly believe this supplement is working for you a urinary 5HIAA test should be done every month or so to see if your blood level of serotonin isn’t too high-if you have any coronary artery disease using this supplement is dangerous there is a whole complex interplay between B vitamins and serotonin as well as its percursors like 5ht -natural supplements are often drugs or co-drugs in a sense so please don’t be over-comfortable by their "natural" label-there are cancerous tumors that secrete gobs of 5ht and that is natural too-for the tumors. Just be careful please LM

Thanks, Tono. Actually the kava kava really does help me. The thing I like to keep in mind is that everyone is different. That’s why I have a hard time beleiving in diagnoses – there are too many variations. I’ll keep your suggestion in mind. But for now it’s the only thing that DOES help.
Well, I’m glad it’s helping. Just be sure not to over do it. As far as the diagnosis, I know what you mean. My Dr. never gave me an actual diagnosis, or I would have 10 or 20 of them! He only tries to work with all my various symptoms. And like Margrove said, BE CAREFULL! Well, in my experience, if you taper off of the drug, much the same as you slowly build up to a dose perscribed by your pdoc, you will experience less side effects. The reason why it seems like you are addicted is because your nervous system has become dependant on the drug, and if you were to just stop it, you will experience severe side effects for several months afterwards, which happened to me a few years ago. However, it’s not anywhere near as severe of an *addiction* as heroine or alcohol. Frost

different drug different mechanism of action-if your gonna do the medusa dance get her to dosie doe a bit and get her permission to up your ativan or up hers and call Jeff Apter-Zoloft compared to remeron is like qualuudes vrs speed so your now on dexamil and doin a bounce-50mg may be too high for a conversion for you it is a transition so make yourself comfortable as possible instead of white knuckling it-that lack of appetite will change to some carb craving soon this is a signal the zoloft is working but imo may be just change for change sake as the more you crave and consume the less you will burn and lose-she is using an older viewpoint that zoloft reduces appetite-ask some others here if their appetite is reduced-it is no less difficult to wean off then remeron in fact it may be a tad bit more difficult due to its energizing effects on some-get a professional opinion by a psychopharm doc not a transplant renal nephrologist whatever—- she wants to maximize the transplants life and that’s ok but there are other issues-including if you are psychologicaly ready to embark on being med free-trade offs is the name of the game-ch ch ch changes—-love bowie within a few days you will feel better be a patient patient but an assertive one-its your kidney now and your medical program- LM

How long are you going to look at that Snickers Bar???? Sweetie, ignore the crap here. No biggie. Now, you know that it will take awhile for the Zoloft to get to a theraputic level in your system. You need to hang on through these few physical adjustments UNLESS they are bothering you too much. Then call the doctor and tell him. If you have a chance, call the pharmacist and ask him to give you the details on the zoloft. He should be able to pop that info on his screen while you are on the phone with him. Let him know the side effects you are having…then he can look it up. When my Effexor was increased, each time I had a strange symptom or side effect, which eventually went away. I hope that your sweats and unsteadiness will not be for long. Is that Snickers still sitting on your hutch? love,Julie

I HAVE a boat…at least until the sale closes. A friend at work boat a repo at auction, and had a great deal of fun getting it back into good condition.
Peter, I spent the last 4 years turning a boat I bought in non-running and unihabitable condition into the boat you now see on her website. In fact, when he was done, he was sort of bored — their wasn’t enough to do.
Which is at least one of the reasons I’ve sold it. He sold it, and is looking for another.
Imagine how bored he’d be if he he no longer goes to work every day either. However, I’m just in transition…exploring opportunities. This too shall pass.

Question:
hi key, that happened for me on effexor and every other a-d i tried including aurorix, celexor, tofranil and luvox. my doc said the effects would wear off over time- but they didn’t. it took great perseverance to get it up, let alone climax. naturally, being a guy, this is hard to take. I was going to say ‘hard to swallow’, but I’m trying to be serious here…. normal sexual response returns within weeks of giving up the a-d. at least it has for me. Erm, Wellbutrin is an a-d that’s known to have a positive effect on sex drive so, if you’re somewhere in the world where its available (its not available in Australia – dang!) you could ask your pdoc to swap you over. hope this helps…

The side effects I experienced were similar to the ones experienced on 20 mg of Paxil. I am now taking 225mg of Effexor XR. I am experiencing delayed ejaculation, it now takes about 20 minuets.

No sex drive and inability to reach orgasm. I had the same side effects. After several months these side effects hadn’t gone away, so my pdoc added Wellbutrin. Much, much better now. :-)
Response:
Classic Effexor side effect. In some people the sex drive comes back. I don’t think the anorgasmia (is that what they call it?) goes away, if it’s one of the side effects for you, until you’re off Effexor. Then everything will be back the way it was before. Let the doc who prescribed it for you know if it’s a big issue for you. Take care. – Hide quoted text — Show quoted text – I’m experiencing loss of sex drive and inability to reach orgasm on 150mg Effexor XR/day. Has anyone had a similar experience and noticed a reduction of these side effects? Basically, will I get my sex drive back? key Before you buy.

buspar and beer….
Question:
Hi Tom, I am a wine-maker and get a lot of pleasure from making and drinking my own wine. Most medications (as far as I have seen), tell you to avoid alcohol (there, that’s my disclaimer <G) but I truly do enjoy a good glass of red 3/4 times a week. Several times I have even had (gasp!) 2 glasses of wine, but I draw the line there. What I have found is that I have no tolerance for alcohol -I either feel very sleepy or get quite giddy. I have not felt any after-effects the next day. My doctor is fully aware of my hobby.
I think most doctors automatically tell you not to drink, or very little, whether you’re on medication or not. But, for me too, when I’m on a benzo, alcohol has an increased effect, and a lot less of it goes a lot farther than I want. A lot of us self-medicate with alcohol as it is a CNS depressant sort of like benzo’s with a much shorter half-life and more dangerous side-effects. Listen to your body and you can probably tell is you can handle a little alcohol with the drugs or not. (Unless you are a substance abuser, that is.) Gordon Held

That seems to be the case. Too bad meds ALL have to say this. I had to take an antibiotic once with the same warning – but there it DID matter. WOW! Talk about am upset stomach! Yikes! So… a beer or two is cool. A good glass of cabernet even better! Cheers… Tom – Hide quoted text — Show quoted text – Hi Tom, I am a wine-maker and get a lot of pleasure from making and drinking my own wine. The times that I do not drink at all are the periods when my anxiety is extremely high and I am either levelling up on a medication or trying a new one or if the depression is very bad. Then I wait for a few months until I feel "balanced" again. BTW, I take a TCA daily and an occasional benzo when needed. If you are concerned about damage to your body, you could have your doctor do a liver function test. Cheers! Charley So… I want to understand… Does taking Buspar or Xanax, etc. forever disallow you from enjoying a beer or two with friends -something that has REALLY lifted my spirits in the past. I’ve battled this question myself several times and usually have the beer or wine, not to excess. But I WOULD like to know if it is TRULY harmful, in this VERY moderate way? Doctors will, of course, say no alcohol. Pharmacy, same thing. I understand. They have to cover their ass and the combination probably is not good (chemically speaking) but alcohol never is good for you anyway. Addictive people should NEVER cross the two! To that I agree. But, once you take some of the "fairly" harmless fun out of your life, your anxiety can go though the roof! I KNOW! Exercise is great. I do it myself regularly when I can. BUT… giving up those few beers with my friends FOREVER seems harsh! I drank twice (half bottle of wine once and a few beers another time) while on Buspar (5 mg/twice day) and Xanax (1 mg 2-3 times a day). All I noticed is I had a good time, might have slept TOO well, and maybe a little more groggy getting up. So what! Took a shower and went back to my daily, fairly healthy grind! Probably a week or two later I may have done the same thing. I have a high tolerance to drugs (Found out the hard way after an appendectomy!). That’s just "my" body chemistry. Just curious if I’m crazy or not? Please no preachy non alcohol speeches. I’d just like to know what the REAL danger here is. Maybe we should all just go to http://www.beer.com and send each other a "virtual" beer! It’s really there! Someone else can start that tradition. Any takers? If you react well to it there should be no problem in moderate drinking, although it’s better not to do it on a daily basis. I often drink a few glasses of wine myself and have done so on 375 different meds and I’m still around with the ol’ liver still intact. *Moderation* is the keyword here though. Interaction with benzos: alcohol and benzos both are CNS depressants which means that they may enhance each other’s effects: in other words, you may get drunk sooner or be more sedated than others. Hangovers may consist of PA’s. I’m not familiar with the exact interaction with Buspar but I do know that except with MAOI’s, AD’s can be taken while drinking a few glasses of wine/beer. My pdoc doesn’t mind me drinking as he knows I’m not going to be a alcoholic. I had a REAL BAD experience when I was a drinker and took Buspar. The two do not mix together well at all. I personally don’t approve of drinking while taking meds…end of story. The physicians have given me zoloft + xanax then prozac. Xanax was horrible…I don’t know how it helps anyone. The zoloft and prozac were similar in effect…made me feel more content and made me much easier to get along with. I didn’t want to be content or congenial, though. I am panting to be more comfortable in college…that’s all. The only relationships that I want are ones in which I am idolized as the enigma that I will always be, whether real or imagined. Doesn’t it make any of you feel empty to act like everyone else? I believe that if I weren’t shy, then I would be suicidal. The biggest obstacle is the feeling that society is attempting to change me, and this becomes especially evident in college. Their attempts wouldn’t be distressing if I wasn’t so sensitive to disapprobation…I want to be liked…not actually liked, too spiritless, but adored, envied, venerated, aggrandized…I don’t want to change. The kind of counseling that I am getting isn’t helping, because I struggle with imagining that anyone really understands me…not because I’m too exceptional to be understood…because I have a distorted way of expressing myself. No offense, but it seems to me that even doctors of the mind are easily misled…One must diffuse what I am saying to get to what is genuinely there. I suppose I am expecting them to read my mind…after all the path from there to here is devious, and I’m not figuring it out. I am mentioning all of this, because most of you talk about a manner of progress that is completely dissimilar to what I imagine for myself. Anyway, we’re all just people experiencing the same symptoms, spawned from varying origins, induced by whatever, with different motivations, resulting in this…however you want to think of it. Also, I felt like being opinionated and declaring that I hate mind playing drugs….the right ones, the wrong ones, the ones that work, the ones that don’t…any of them. The ones that play in the pain region of your brain (no one expects me to know the names of all of these chemicals, regions, nerve transmitters, right? is it with the prostaglandins…I don’t know.) and other similar places don’t offend me that much….it’s just those ones that play in your serotonin governors, emotional signals, GABAs, monoamine oxidase, whatever. I’m not denying these drugs’ rights to excitement…They’re just uninvited right now. Gotta be elaborate about it, y’know.

What else do we have in common?
Question:
Hi group, the question about acid reflux got me thinking. It seems to me that quite a few in this NG have other medical problems besides migraines. Are migraineurs as a group more susceptible to other problems? Are our systems weakened by the chronic pain so that we are not able to fight off other illnesses as well? Or are other illnesses present that pre-dispose us to migraines? If you all are interested, I’d be willing to compile answers in a spreadsheet format. If you feel that’s not appropriate let me know. Otherwise, e-mail me personally and I’ll get going on this. Obviously, I don’t need names for this, just illnesses. What do you think? Ingrid
Ingrid, most of your questions I can’t answer and wouldn’t try to for the group but I cn tell you that most of my health probloms seratonin plays a major factor in. Don’t knpe if that helps or not Karen

Jane, I had never heard of geographic tongue either, so I looked it up in my handy-dandy Taber’s Medical Dictionary: One possessing white, raised areas resembling mountain ranges on a relief map. Areas consist of heaped-up epithelium surrounding areas of atrophy. The next entry after geographic tongue was hairy tongue. Hmmmm, hairy tongue vs. geograpic tongue! Both seem kind of nasty, in my book. Mary – Hide quoted text — Show quoted text – Ingrid, I nver get sick. I nver get the flu. Hi Ingrid, Don’t think I am unusual (except for being unique like all humans) but only have migraine. Don’t seem to have any other chronic conditions and have otherwise been healthy all my life and am still fighting fit and functioning on all levels at 66. Isn’t it possible that these other conditions, digestive, visual, auditory etc are all part of a wider migraine syndrome. Is there a specific thing called migraine, or is it an umbrella term which still defies close definition? Don’t have the answer..just ask the question!!

LOL! They printed off an ID card & everything, waiting for him to turn up and verify his identity. But they sent his application off to the accomodation office, with a note saying present accomodation should suit him fine
Then at the end of the year the lecturer of one of the courses he’d "enrolled" in had notices up asking him to get in touch ASAP, because he’d been handing in work – and getting it marked! – but wasn’t on the official class list! </ramble , but lower than normal BP (at 90/60 on a good day). I also have cold hands always, even in the 122 degree heat Phoenix is famous for, and even with the raised body temp.
My BP’s always pretty low (dizziness, etc.), temperature I don’t know about. And I’d have to convert it to fahrenheit anyway, wouldn’t I? butting (t’ lazy) — Bryce Utting http://www.cs.waikato.ac.nz/~butting the cross before me, the world behind me no turning back
