Question:
Hi Relating to Medic Alert bracelets: Does anyone else wear them? I have moderate to severe asthma, and am on periodic bursts of prednisone, but not on any maintainance doses. I also use flovent 220 4 puffs a day. Does anyone think this would be a situation requiring one – and if so what would it say? Asthma, corticosteroid use? I don’t have a clue, although some friends have suggested I should wear one. Thanks for any help. Sherry

I wear a Medic Alert necklace, with the medallion hanging in back. The engraving just says ‘asthma’ plus an ID # plus a phone # to call for more info. My personal info and drug useage is on file with Medic Alert. I am on Low dose inhaled steroids (6 pf Vanceril/day). Your dose of Flovent 220 is in the High dose category (3 pf Flovent 220/day) so I would be inclined to add ’steroid dependent’. You are also considered steroid dependent if you have used oral steroids in the last 6-12 mo. The tag could be very useful to emergency personnel if you were in an accident. Ellis

I’ve got one. My asthma isn’t all that bad, but my parents would prefer that if I end up in an emergency situation, the doctors know that I have asthma and to be aware of that fact. Of course, I’m also allergic to any form of penicillian and any sulfa-type antibiotics, so that’s on there too. I guess all of these are pretty important, so that was my Christmas present last year. When you fill out their paperwork, they ask what medications you take and who your doctors are, along with who to contact. Personally, being away at college (for the next 2 years, anyway) and not having family close, I kind of appreciate the thouroghness of the system. If I were in an accident, or needed emergency care, the bracelet knows more than my friends, and probably more than my parents would remember

Hi, new here. A little background first: I was diagnosed with mild asthma in August and was given an Rx for albuterol 2 puffs as needed. At first I was hardly using it, maybe once a week, then I started going to the gym and as advised by my Dr I’d use it prior to working out to prevent an exercise induced attack I stopped going to the gym for awhile and just started up again 3 weeks ago. In the last week and a half I’ve been getting chest pains much like what sent me to the Dr. in the first place as well as "flutters" in my chest. I noticed that my inhaler use has gone way up, its almost daily now and some days twice and most of the time I use it and it does nothing. I plan on calling the Dr. in the morning to make an appointment, but I’m just wondering if this is normal? Other then being constantly sleepy (which I think is in part due to being unable to breathe well and part due to jitteryness from the inhaler) and the chest tightness I feel fine, no fever or anything… Sheila Marie

I have both Pulmicort and Singulair now. The Dr. is hoping we can get the attack under control and drop the Pulmicort in a month or so. Sheila Marie, still not quite right, but feeling better.

Went today, I was scared last night, I’ve never felt anything like that before. I almost thought for sure I was having a heart attack at 28, but it wasnt. The Dr sent me home all sorts of medicine. He said we need to be more aggressive in my treatment. Seems exercise is a major trigger for me, though its one I need to find a way to get past or something. That’s the goal though, to get to a point where exercise doesn’t trigger this sort of thing. Sheila Marie

When I first started going downhill with my asthma my main complaint was being extremely tired all the time and the inhaler just sustained me but did not make me feel better. You probably need to be on a low dose of Flovent to start and probably Accolate. Go see your doc! And if he doesn’t change your meds than go to another doctor b/c your asthma will only worsen w/o proper treatment!! Dana

Hello, I am new to this group and hope to find some support for my panic attacks. I used zoloft for 10 years to control panic, and have been off it for 1 year. (I tapered off very slowly over the course of a year.) Now panic attacks are returning in full force. I am trying to control them with gaba, but not having much success. Does anyone have any thoughts or information on wjether the brain can recover after 10 years of anti-depressant use? Or suggestions for alternative approaches. I am desperate because my anxiety is leading to diahrea, lack of sleep, loss of weight. Thanks for any help.
Gaba doesn’t pass the blood-brain barrier and anyway it’s not a *lack of Gaba* that’s causing PD. I would suggest trying a benzodiazepine like Xanax, Ativan, Klonopin, Valium which, unlike the antidepressants, *do* work on the Gaba system. Benzos will cause *dependence* (as do many ADs and a myriad of other meds) for which reason some doctors won’t prescribe them, confusing dependence and addiction. Benzos are first choice meds for panic and dependence means you shouldn’t stop taking them suddenly but by way of a slow taper. Let me assure you that what you are experiencing is quite normal for people with anxiety disorders, meds or not. Setbacks after the cessation of medication is unfortunately…. too common. I urge you to seek professional help as soon as possible. If you are very adverse to taking another antidepressant, than ask your doctor about benzodiazepines. If you are against meds period, then look into cognitive behavioral therapy. It is the most effective therapy for anxiety disorders. I can see your problem,but (this maybe doesn’t is the info you are looking for) I think the problem isn’t the intake of zoloft for 10 years. When I read your symptoms I can only assume your PAD isn’t under control without medication. I don’t think your brain needs to recover from Zoloft. Jackie gave you some excellent links. Unfortenatly PAD isn’t a disorder that disappears. With very few people it is a temporary thing. Of course you are free to live without medication

I am coming off of Serzone after using it for at least 10 years. I came off the Serzone very slowly and have medical supervision on this. I have been told by my Dr. that if I was to come off the medication too quickly, I would probably trigger an episode of deep depression. My experience so far now that I am off Serzone completely for 3 days is that my tension migraines are a wee be worse but my head has lost much of its "fog." Long term use of the SSRIs presents a lot of problems. It doesn’t usually work forever and at a certain point they need to be stopped. As an episodic problem panic disorder probably is going to do best with an episodic treatment. Withdrawal after long term use can be much more severe than you describe, and people need to be aware that withdrawal induced suicidal ideation is a problem — as is the induction of new symptoms of anxiety and panic that were not present previously. Zaps are often confused with panic attacks. So is withdrawal related visual lag/vertigo and withdrawal related nausea. All that aside, what can one do? It seems that when a person stops a SSRI, they are relatively serotonin depleted. My strategy to boos serotonin on the supply side is to prescribe l-tryptophan at a dose of 1000 mg twice daily — to be taken on an empty stomach with a bit of sugar only (juice, soda crackers, etc.) Tryptophan is converted to serotonin in the brain. I avoid 5-HTP because it is converted into serotonin to a large extent outside of the brain and this can potentially cause the same sort of cardiac problems that phen-fen caused. Taking the tryptophan with SAM-e 200 to 400 mg can boost serotonin synthesis even further. If you have a prior history of manic episodes, avoid the SAM-e. Some of my patients have found significant relief from SSRI/Zoloft withdrawal using this strategy. Hi Laura, It sounds like the Zoloft was controlling your anxiety disorder, and after you stopped it, the panic attacks returned. I’d restart the Zoloft. I’ve been on Zoloft for over 10 years, primarily for recurrant depression. In the past when I have discontinued the Zoloft, my depression returns. Welcome, Laura!! I’m sorry you are experiencing this again. There is no reason you can’t resume your Zoloft if it helped you before, and you need it. I don’t think you are ‘recovering’ from 10 years of anti-depressant use…I think you are just plain suffering from an anxiety disorder. It’s really OK to take meds if you need them, Laura. A diabetic needs insulin…someone with anxiety or depression also needs meds. It’s the same thing. If I were you, I would contact the doctor and start something again. sylvain, (&FJ if you’re reading this) been out of town for almost a week, hence the delayed reply. i switched to lexapro at the instigation of my female companion, to reduce the well-known sexual side-effect. in my case, the sexual side-effect was greatly reduced in fact.
That the sexual sideeffects are reduced so much compared th seroxat is good news to me. This is what I’m after, a lessening of sideeffects and the same benefits anxiety wise. Could you make an immediate shift between the two meds or did you have to cut down the milligrams slowly and then start on the new med?

Usually you should be able to cut down one med and up the other at the same time but discuss it with your shrink.

i actually stopped taking the paxil completely & switched to lexapro the day after i got some. i wasn’t as worried about that as i perhaps should have been, because several times before i had missed one dose of paxil–but made sure i never missed two in consecutive days–and missing one dose didn’t affect me greatly, just made me a little edgy.

At times I get all sad if I only miss my Effexor dose by a few hours. At other times I can miss it completely for a day and don’t notice. It’s weird.

If you’re on 20 Mg, a raise to 40 Mg should do the trick I’ve been on Seroxat (Paxil) for 5-6 years. Sylvain. Hi Sylvain! Does this mean that you have to raise the milligrams again and again, untill the med doesn’t work anymore?
FJ. According to the manufacturer; 20 Mg is the minimum therapeutic dosage for SP, and 50 Mg is the maximum. If you get no improvement on 40 Mg, I don’t believe that there’s much point raising it to 50 Mg. It’s 60 Mg max. for OCD, by the way. I would urge you to stick with Seroxat as long as possible, as you just don’t know how the other meds. will work with you. On the other hand, if you want badly enough to change, by all means do so. You could try Celexa (Cipramil, Citalopram) first. Sylvain.

Hard to say, really. They all fight SP to some extent but their effectiveness is highly dependent on the individual. Effexor works quite nicely for me and simply fantastic when it comes to my depression.

I am assistant to a professor and go back out for teaching this season (next month) if can get energy up . . . could not handle effexor withdrawal so I put myself back on this weekend. I need my zyprexa, but can’t have it. Side effects put me in this damn wheel chair. Not a single friend here. I know Aware here, but not together enough to ask to meet yet. Welcome to ASD. I am in the minority concerning sleep habits. When I go into "the pit," I become hypersomniac. A couple years ago, I went through a stretch in which I slept 14-15 hours per day. Of course, the waking hours were living hell. Plus, that much sleep (fitful sleep, I might add) is disorienting. It seems to have the end effect of no sleep at all. Anyway, I hope you get some sleep. Hi, I’ve been a low-fat type person for a long time. The last six months I’ve been on Paxil and have gained some weight. The low fat diet isn’t helping me. Hi Dosbabe! I’ve been on Prosac for over a decade, happily, and have had some good results. I not one of the fastest losers, but I’ve managed to take off about 50 pounds. Yeah I’m taking zoloft and doing fine. According to Schwarzbein (TheSchwarzbein Principle), A low carb diet will help balance out the chemical soup inside your head. She claims insulin resistance= high carb diet, is one of the things that unbalance and deplete serotonin in your brain. Which is the same thing that antidepressants try and balance. Give it a try. After talking with your Doc. first.

Funny you mention that, because I missed my Effexor dose this morning and am curious as to what the results are going to be. Only been on it for about 2 months so may not be too noticeable. I was on Prozac when I first started just over 3 years ago. It didn’t hinder my losing 60 pounds. I tried Imiprimine last month for help with sleep as I have fibromyalgia. I couldn’t take it as it made me sweat and also caused terrible sweets cravings. I’ve been on Trazodone for 10 days and am not having any problems so far. Good Luck. TamH in VT

My daughter was also on Paxil and gained weight she didn’t need. She stopped using Paxil and went to Prozac and has had no problems and lost all the weight she gained. I’ve been on Prozac since 1989 and haven’t had any problems with it either. I gained a lot of weight when I stopped smoking (50lbs) and being sick and inactive for so long. Well, I took effexor for 3 days and slept like in a coma. Could not stay awake. When I did finally manage to awaken, I walked around like a zombie with a spinning head. Celexa may have less effect on your libido, or it may be worse. The only way to find out is to try it. In most cases you can switch between SSRIs (and SSRIs to TCAs, SSNRIs) overnight. But there are other things you could try first. Do a search of ASAP’s archives at Deja (or whatever they are now called) for " sexual dysfunction ". Best wishes Ian

Hi Cheryl, I know how you feel. Supposedly, switching from Paxil to Zoloft was not supposed to be any big deal, but I feel different and it is not all my doing. I am more nervous, and have the trembles. I am just trying to be patient and see if it passes with time. I still function, but do not feel comfortable. How long are you going to look at that Snickers Bar???? Sweetie, ignore the crap here. No biggie. Now, you know that it will take awhile for the Zoloft to get to a theraputic level in your system. You need to hang on through these few physical adjustments UNLESS they are bothering you too much. Then call the doctor and tell him. If you have a chance, call the pharmacist and ask him to give you the details on the zoloft. He should be able to pop that info on his screen while you are on the phone with him. Let him know the side effects you are having…then he can look it up. When my Effexor was increased, each time I had a strange symptom or side effect, which eventually went away. I hope that your sweats and unsteadiness will not be for long. Is that Snickers still sitting on your hutch? love,Julie

While clonazepam seems to be the treatment of choice, I don’t see why it would be better than any other benzo. There are many, many treatments for RLS, as is often the case when no one treatment is that successful. I have just looked up information about restless leg syndrome this morning. I checked the sleepmed site and National Sleep Foundation. Apparently, there is a correlation with B-12 deficiancy and rls. I had restless legs last year and before.I started to improve my eating,exercising and I started taking vitamins and minerals related to stress and anxiety. I started taking the B complex with a B12 and I have not had any problems with restless legs for about a year now! It is a vitamin, that I buy at WalMart. I would give it a try as it will not hurt you nor interfer with prescriptions. Facts About Restless Legs What is Restless Legs Syndrome? If you have restless legs syndrome (RLS), or are a physician seeing a patient with RLS, you’ll probably recognize these symptoms: An urge to move the legs, often accompanied by uncomfortable sensations in the legs, usually described as a creeping or crawling feeling, but sometimes as a tingling, cramping, burning or just plain pain. Some patients have no definite sensation, except for the need to move. (The arms may also be affected, but that’s much less common.) The need to move the legs to relieve the discomfort, by stretching or bending, rubbing the legs, tossing or turning in bed, or getting up and pacing the floor. Moving usually offers some temporary relief of symptoms. A definite worsening of the discomfort when lying down, especially when you’re trying to fall asleep at night, or during other forms of inactivity, including just sitting. A tendency to experience the most discomfort late in the day and at night. Sleep disturbances are common with RLS, primarily because of the difficulty it causes in getting to sleep. If leg twitching or jerking is also present, a related disorder called periodic limb movements during sleep (PLMS) may be the cause. With PLMS, the leg movements may be severe enough to awaken you (see the PLMS fact sheet). In RLS, PLMS-like sypmtoms can sometimes occur during wakefulness, as well as in sleep. How common is RLS? Restless leg syndrome may affect as much as 2-5 percent of the population, with varying degrees of intensity. What causes RLS? The cause of RLS is still unknown. Some cases are inherited and more than one family member may be affected. Some cases have been associated with nerve damage in the legs due to diabetes, kidney problems or alcoholism. RLS can also be a side effect of a pinched nerve root from arthritis in the lower back (sciatica). Is RLS serious? RLS is not considered medically serious. However, the symptoms can range anywhere from bothersome to incapacitating. Fluctuations in severity are common, and occasionally the symptoms may disappear for periods of time. RLS can begin at any age, but the symptoms tend to worsen over the years and become more severe in middle-to-old age. Pregnancy or hormonal changes may temporarily worsen RLS symptoms. Stress, diet or other environmental factors may play a role for some people. Can RLS be treated? Most cases of RLS respond well to medical treatment. There are three main classes of medication that have been shown to be effective in treating both RLS and PLMS. Benzodiazepines – This class includes such drugs as diazepam (Valium),clonazepam (Klonopin), temazepam (Restoril) and triazolam (Halcion). The nuerons of the muscles are still firing to one level or another in a hyperactive way-you can notice it most when you are overtired. Some are helped by adding a dose of niacin or niacinamide a b vitimin around 100mg to their diet-others a nice physical workout 3x per week does the trick. There are vascular problems that can mimic this benign condition so if you have pain or numness coldness or severe cramping of the calf muscles have your doc check it out. Some folks are also helped by increased calcium and magnesium but ask a doc first. LM

Just wanted to say thanks for this info…we had a pt. at the clinic where I med. assisted in the early 80’s with RLS, and I felt so bad for him, since there wasn’t much known about the cause, tx, etc. Now I’ll have something to refer to if I encounter someone w/ the problem again… Very interesting & helpful, as always! An increase in my Zoloft dose once resulted in a worsening of my RLS, and this got better after a reduction in Zoloft dose. A later increase in Zoloft dose did not cause a worsening of my RLS. A temporary reduction in Zoloft dose may resolve this problem for you. Good luck. Chip

I have "periodic limb movement syndrome" which is different in the effect that my limbs will twitch or jolt out uncontrollably; not too much of a pain, I’m pretty used to it; and it isn’t that noticable unless it’s noghttime. Right before I fall asleep my legs jolt big time! I’ve pretty much learned to pay no mind to it. It’s as subconscience as breathing to me, now.

My pdoc who has been diagnosed with this syndrome treats it by taking a very low dosage of Klonopin before bed, since that is when it is most bothersome. However I have heard that some people can gain relief through vitamins, but I really do not know much about that, since this is not something I have done much research into. Good Luck! d

I thought I’d refine your post, because it’s really not fair to characterize prozac or other ad’s as contrib- uting to suicidal ideation. Medications that have such warnings usually do so to remind the physician that their patient may be suicidal; in fact, some medications — particularly those that can be overdosed — make this warning very clear so as to alert the physician to the possibility of using the medication itself as an instrument of suicide. C/

Serzone was the last anti-depressant I took last January through July. I have taken zolft, prozac, effexor, and trazodone when it comes to anti-depressants. Of all of them Effexor has worked the best. I was also on Serzone for several months and found myself tearful most of the time,almost suicidal. I talked with my Md. and was changed to Celexa. I have found myself much better. I posted this before http://www.ozemail.com.au/~drobb/bms/pi.htm for info on Serzone. It has some interesting things to say. I use Valium when I need it and passed on the offers of Zoloft, Xanax, Prozac, and the like. I was already aware, from first hand observation of those taking them, of what they are capable of. I will stay with what I have, thank you. It may not be popular but it seems to be a whole lot safer if you don’t need the effects that the others bring. It may not be the best solution but it is one that works for me to take the edge off when needed. The best part is that I was allowed to use the minimum dosage and when I need it. The hard part is trying to get through those times when you may want to take it but you know the feelings will pass faster than the pill can take effect so you just tough it out. If you can do that, you have half the battle won. Good Luck.

If you are hypo-thyroid and feeling suicidal 1) make sure your thyroid hormone level is adequate 2) get switched onto a T4/T3 combo And just because a TSH test was done and the numbers are in "the normal range" does NOT mean you are not hypo- there are problems with how the TSH normal range is calibrated, most MDs are not aware of the issues and questions about this. They are taught that the TSH test is definite, accurate and extremely sensitive — sensitive it is, but the accuracy depends upon how it is interpreted (not blindly) and it can never be definite — because there are forms of thyroid disease where TSH numbers become meaningless. T3 is one of the best anti-depressants in the shinks arsenal, most MDs however ignore or don’t know this. Depression and suicidal feelings are a symptom of hypo-, you may be undermedicated for hypo- or mis-diagnosed as NOT hypo- when you are. Karen, The only idvise i can give, see your dokter. Efexor helped me get over the suicadal thoughts. taking now 300 mg, two times 150 mg. Kevin

You know, suicidal thoughts occur to a lot of people a lot of the time. There are days when it is a thought and something distracts you and you go about your business until the next time. Depression doesn’t help things either but staying busy and having an active social life is a tremendous help. Most of us here have had bouts with depression ( some severe and some accompanying PTSS ). My heart goes out to you. Please feel free to write if the need arises. I have found a link to the product information on Serzone. It may help you ( or even the others here to help ) you understand better how it works and if you should discuss it at length with your doctor ( my personal opinion from experience is to talk to them at length about it ). The link is http://www.ozemail.com.au/~drobb/bms/pi.htm and it goes into some depth so assistance from someone like Ted or one of the others that are more medically or chemically knowledgeable may be needed to understand it better and maybe explain it all to you in lay terms ( Lord knows I don’t understand it all ). If the thoughts of suicide become too much I suggest you get a hold of a friend and have them take you to the hospital for observation until it passes. Life is too short and precious to waste on a disease that is treatable. The hospital cannot keep you if you wish to leave so you shouldn’t worry about that, but, having caring people available with the knowledge to help will save you from doing anything rash. It would also be of benefit if a different type of medication needed in it’s place. From all that I can gather on Ativan, it is in the same family as Valium and has potential for dependence. Personally, I take Valium in as small a dose as possible when needed. I wouldn’t, however, discontinue use of your medication under any circumstance unless it is under observation and approval by a health professional. ( this doesn’t mean it has to be in a hospital but your doctor does need to know if you are discontiuing use as potential for withdrawl symptoms can cause complications ). Yes, you have a couple of issues that need to be resolved. I found that as the level of thyroid medication was increased I needed less and less of the Valium for the anxiety attack ( of course mine are PTSS related ). I’ve had to learn that they will pass in time and that I will be okay ( I can feel when they are coming on so I take a small dose of Valium at the onset ). Even without the Valium, I know it will pass but I try to stay active during the episodes ( it seems to help ). After learning to cope, the episodes tend to diminish in intensity when they do happen and so they happen less frequently than they used to. You may find you have similar result or find something else that works for you. In either case you need to talk to your doctor about it and discuss your options. Always remember you have people you can talk to and if nothing else, I monitor this group and my own mail regularly. Feel free to write if you need. You are not alone, even when you are by yourself. All you need to do is let someone know.

A month and a half is just enough time to get onto the first rung, re: dose. Since his dose is not working well enough, ask your doctor about another step up. It is a long process to find the right med and the right dose… but it sounds like it may be too early to give up on this one yet. If you are having suicidal thoughts, the important thing is to NOT be alone, while your meds are adjusted and stabilize (which can take many weeks). Don’t be discouraged…. but also, don’t be alone until you are at a dose that works well enough. I was on serzone for 8 months over which i went from 50 mg/day up to 600mg/day. I liked serzone re: lack of adverse side effects, but it just wasn’t enough for me. It was thoughts (and detailed plans) of suicide at the 600mg/day level that got me to change (i.e. give up). The difficult part in any change is the interface, the drop out from one med, the drop in of the other. If you reach a point where you need to change meds, do not trust yourself being alone during this period. Ask a close friend to ride with you. Or if all else fails, the hospital is there to ensure that you are protected during these transitions. There is no shame. The right medication will help, and you will be glad, and safe, and free from the suicidal thoughts…. once you find it. Again, I think it may be too early for you to give up on the serzone yet… try higher doses with your pdoc (what dose are you on now?). For many people serzone is very effective, and it’s the one they stay with…. once the find the right dose. best wishes, Han
