I know that not gargling after taking inhaled steroids can lead to problems. I was just wondering if taking inhaled steroids and having oral sex could cause a problems too. Would there be an increased chance of getting an infection? Thanks

I’m really not sure what kind of infection, I just know that my throat doesn’t feel right and my Asthma gets worse. The doctor says it looks really red and that I could have an infection or just allergies. They usually just give me antibiotics, I don’t think they’ve ever taken a culture.  I asked the question only because the problem seems to occur around the same time I’ve had this kind of activity and I wanted to know if there could be a connection.  Maybe next time I’ll insist that the doctor take a throat culture.

Interesting post I read here but no one expanded on the subject.  What is the opinion of inhaled steroids and weight gain?  I would like every ones opinion whether weight actually increases using inhaled vs oral steroids.

Weight gain is a common side effect of oral steroids.  The chances of a steroid side effect is proportional to the dose – the greater the dose, the greater the chance of side effects.  IIRC the rule of thumb is that once the daily dose of inhaled steroid goes over 1000 micrograms, there is a chance of side effects. However as in all things relating to asthma and asthma medications: ‘Your milage may vary.’ "…there is always a well-known solution to every human problem–neat, plausible, and wrong."    H. L. Oral steriods like Prednisone definitely cause sudden weight gain and haven’t heard any convincing evidence that inhaled steriods do not. I have put on considerable weight since on Flovent 220 mcg and wonder if it might be to blame. Anyone else? Dana

I take Advair, in the lowest dose, but I have lost weight.

I’m on 2000mcg inhaled steroid per day with short weekly courses of 30mg every day when required (usually every two / three months or so). Much as I’d like to blame my weight gain on the above, and I’m sure it does play some small part, I think the main reason for being my being overweight whilst taking steroids is that I find it hard to exercise without getting out of breath and wheezy, get depressed from not being as active as I’d like, and hence use food as comfort. I’m kidding myself if I think I can just blame it on the steroids.

The Concerta web page says that if you take 15 mg Methylphenidate 2x/day or 15 mg Methylphenidate 3x/day or 60 mg Methylphenidate-SR, then you should take  54 mg of Concerta every morning by taking a 36 mg and an 18 mg at the same time. When a new med is developed, that company that developed it has exclusive rights to market it, as with any patented invention. After, I think, 20 or 30 years or so, they lose the exclusivity and other products can copy their design/formula. They can also "lease" the right to use the patented formula/design during their exclusive rights period. ALZA’s web site says that their OROS delivery system has been used for 20 years, but that Concerta uses an enhanced OROS system, so the clock may have restarted ticking, it may be like a new patent. Most health/medical insurance policies push generics (list of covered meds). When my daughter was on Ritalin, I had to make certain the doctor put "Brand name medically necessary" on every prescription, or our previous insurance company would only pay for the generic, since it was available. To get our new insurance company to pay for Concerta, we had to jump through some hoops. The doctor had to write a letter (for which I had to provide him information since we’re new to the practice), detailing all the meds daughter has been on, why they weren’t effective or were problematic, and why we think Concerta will be different. Since daughter’s tried everything, they approved our request and are paying for Concerta. They did not approve it for son, however, as he hasn’t tried Ritalin-SR (much cheaper than Concerta and theoretically similar). Patents give companies (or people) the exclusive right to construct or sell or use a process or principle of operation. Well, sorta.  The "game", I am told by folks who have cause to know such things, is to tell enough to get the patent but not enough that someone can reproduce the technology.  Georgia Tech has a course in which you are given a patent and required to design a working device based on that patent.  It very quickly becomes evident how much information is _not_ included in the patent.  Of course this assumes that the device described by the patent will actually work.  One engineer I knew collected whacky patents for stuff that couldn’t possibly work–had his office wallpapered with them. We switched daughter back to Adderall this morning largely based on what daughter was telling us. It just wasn’t working for her. Now we her taking 30 long however I wonder if she can take another 20 mg dose around 2PM? Is there an Adderall website?? Is anyone taking Adderall this way, three doses a day? Last time we talked to her doctor we found that she was splitting her afternoon dose by taking 10 mg at lunch plus 10 mg later in the afternoon which the the lower split dose. Concerta does not appear in our Libraries 2000 PDR and of course although I have to talk to her psychiatrist before we increase (ie to get a script) I was wondering if increasing the dose was an option. I don’t think she gets the kick (maybe poor choice of words) that she got from the Adderall. The Concerta is probably much slower and more gradual delivery and change in the blood level.

That’s how we’ve switched dosages here. Son went from 5mg Ritalin 3x/day to 5mg Adderall 2x/day (but really needing a 3rd dose too late to give it to him) to 18mg Concerta 1x/day. Daughter went from 20mg Ritalin-SR in the AM plus 10mg Ritalin regular in the PM (and still never being stable in the meds’ effects) to 10mg Adderall 2x/day (but really needing a third dose too late to give it to her) to 36mg Concerta 1x/day. That’s how our pediatrician explained it to us.

Well, the med in Concerta is methylphenidate HCl (a form of Ritalin), so it’s already a generic. The delivery system in the body is what is unique and what makes it so expensive. It’s called OROS and was developed by ALZA. It’s been used to deliver other medications for about 20 years, although it’s been enhanced for Concerta to give an intial "shot" of medication. From what our pediatrician told us about the conversion from Ritalin to Concerta, you would need 2 36mg caplets in the morning to receive the equivalent of your current 60mg/day of Ritalin.  (18mg Concerta = 15mg {3 5mg doses} Ritalin) I am at a total loss as to why there is no good indicators of what an adult should be taking for Concerta, if (s)he is coming from a regular release Ritalin dosage of 60mg per day? Taking 2 36mg. Concerta might be considered to be too high of a dose. There is a new 54mg coming out (maybe out now, possibly). But would this be enough?

With the conversion above, I don’t see how 2 36mg would be considered too high.  It’s an equivalent dosage to what you are currently taking. While my USP DI is a bit old (1996, they ain’t cheap), the usual pediatric dose for 6 years of age and older for Adderall, states that the dose does not usually exceed 40 mg daily. Concerta is not listed, as it is newer than my book.

I saw a company chart comparing 18 mg concerta with 5 mg Ritalin taken three times a day. I must say that I did not find Simple Green very good as a degreaser.  I sprayed it full strength on the chain rings and several areas of the frame with no luck at cutting the grease even after vigorous rubbing.  I even let it sit for a while.  I went back to the citrus degreaser which works great. Mark

I find Simple Green ($7 for a whole gallon at Sam’s Club — and you can dilute it and use it as a general household cleanser) works great in my Vetta Chain Cleaner, but really is not that effective on heavy degreasing jobs, such as cleaning headset bearings in races.  Still, it’s a relative bargain compared to many bicycle-specific products. I get mine at the local junkyard. Usually about $5 each. if you just want to disable them, remove the actuator arm(probably easier to pull the motor out).

Any stereo shop that installs alarms will have generic actuators that I’m sure you can make work in place of the Ford units. I doubt these aftermarket ones are better than the stock Ford pieces though. My experience with them is that they can be very hard to turn with the key, unless of course they’re hooked into the alarm and you never have to use the key… Tom Shaver Powerdyne + TW

You can easily remove them if you don’t want them installed or if their causing problems with the normal operation of the key unlocking/locking the door from the outside.  You’ll have to remove the door panels.  They’re fastened in a plastic brace by little rubber boots.  Once you see the actuator you’ll notice how easy it is to remove.  If you want to replace them but don’t want to pay FORDMOCO’s price for new ones I replaced mine by going to a junk yard/spare parts yard and they found some for me for 20 bucks a piece.  Works great! Like new.

    I’ve recently started taking Serevent and Flovent. In spite of rinsing my mouth after each use I have some rough, whitish, patches in my mouth. Can this be thrush? My Dr. prescribed antibiotics but it seems to be getting worse and now I’m wondering… If this is Thrush, isn’t it related to yeast infection? I know that antibiotics can cause yeast infections. I need to call the Dr. back this morning but I’m wondering if it’s time for a new Dr…. Debi

I’ve had Asthma for years and years.  The inhalers can cause thrush and I’ve had it a time or two. I’ve got a habit of taking the inhalers without actually getting out of bed in the middle of the night.  Once I took the pills that they give you for other yeast infections when it was fairly severe.  Usually There is a mouthwash that is prescribed that will kill off the yeast.  (Also when I’ve had thrush it’s been more in by thriat than in my mouth.)  Also, I’ve used the inhalers for 20+ years and have only had thrush maybe four times. Also, take it easy on your mouth.  A sip of water or swish is enough. Your mouth could be irritated from over brushing or from too harsh toothpaste and mouthwash ingredients.  The worst mouth problems I’ve had were caused by an anti-tartar mouthwash my former dentist pushed. My current dentist says lots of prople can’t tolerate them. Jo Before you buy.

That may help in preventing a Thrush episode form occuring but will do nothing to control it once it’s begun. What else should I be doing?    I’m still waiting for a call back from the Dr.’s office.

You need to be seen by a medical professional as quickly as possible.  The longer a Thrush outbreak is left unattended the more difficult it is to place into remission.  If your regular health care professional can’t see you, recommend a covering physician or prescribe something over the phone, for whatever reason, then proceed to seek help elsewhere…. maybe your local hospital ER.  The white patches seen inside your mouth are not so problematic as those not so easily seen, those deeper down in your throat. Anyone here ever get oral thrush from using Albuterol? I have had a devil of a time.  Coated toungue and roof-of-mouth sores.  Also some kind of virus (feels like a cold) which has knocked me out. I have "exercise-induced" asthma (reactive airwaves) and take a few puffs of Albuterol before walking on treadmill, etc. This is the only thing I can think that could have caused the thrush (yes, I know I "should" have been rinsing my mouth out) and not rinsing after has been a hard lesson to learn. Doc gave me Nystain and Diflucan.  Seems to be clearing up, but I feel like crap.  Not sure if that is a result of the meds or the virus. We started adding Acidophilus tablets to our daughters milk every morning. It’s a capsule so it can be opened and dumped.  I’m sure an adult can just take it like a regular pill. It adds the normal flora back to the body that can help prevent yeast infections.  It’s worked well to keep down the thrush in her mouth as well as the yeast infections in her privates. Albuterol does nothing to affect the oral flora and is not an immune modulator and thus cannot cause thrush. I don’ think that thrush makes one systemically ill but I am not certain of this. Other things can of course.

Because this is the first prescription I’ve needed for the baby, the pharmacy needs my insurance card and of course the pharmacy (CVS) where the prescription was called in to doesn’t do deliveries :o ( For anybody else out there who might be interested in the home remedy, here it is: 1/4 tsp. of Baking Soda 1 to 2 drops of Ivory Liquid (YES, dish soap… almost any brand of the liquid dish soaps will do) 8 ounces of water Mix thoroughly and apply (with a soft face cloth) to affected areas. So far so good, almost immediately at his next feeding he was eating better and I am making sure that I rinse myself off after each nursing session so we don’t keep passing it back and forth! — Jackie M. My pdoc tells me that mood stabilizers help with manic stages more than depression, and that anti-depressants are needed for the down times of bipolar. Hope that helps! Can SSRI’s such as Prozac and Effexor help with moodyness and a bit of bi- polar? Mood stabilizers help with moods sure, but what about helping the depression?

You are probably right, Randee. A lot of shrinks have their own shrinks!!  And you probably know that they have the highest suicide rate of any of the medical specialities (the stats for docs generally are also higher than the general population). I just came from seeing him and he told me starting today to up my zoloft from 50mgs to 100mgs.He said I was going too slow thats why I don’t feel better yet.I think i’m doing just fine and i’m going to stay at 50mgs for now and find a new doc.

generally, an increase from 50 to 100mg is easier than 0 to 50, but the real determining factor should be how you feel about increasing, not how your doc feels. I tried to tell him why I was weening slow and he told me there is no risk or side-effects by upping it so high.

Did you start rolling on the floor laughing?  I would have. So I asked him if he ever took an SSRI. He didn’t like that one bit..lol

ADs when taken by those without a problem seem to have very few effects beyond S.Es, so IMO all doctors should be required to take a short course of one. This would do wonders for their care of patients! At least they are calling my insurance company today and trying to get zoloft approved.That would be a huge help.I’m still in good spirits though.Not many side-effects on 50mgs. I’m hoping to be breathing better soon.                                        Randee

You can’t go too slow. Why do these docs act like they have a plane to catch? If you’re feeling ready to raise the Zoloft dose, raise it to 75 mgs first, there is no hurry and your own pace is the best pace. For most people 50 mgs won’t do the trick but if you feel OK there is no reason to do anything about it….. .I think i’m doing just fine and i’m going to stay at 50mgs for now and find a new doc. Oh yeah…… .So I asked him if he ever took an SSRI. He didn’t like that one bit..lol At least they are calling my insurance company today and trying to get zoloft approved.That would be a huge help.I’m still in good spirits though.Not many side-effects on 50mgs. I’m hoping to be breathing better soon.

   I seriously think my pd doc. is nuts.I just came from seeing him and he told me starting today to up my zoloft from 50mgs to 100mgs.He said I was going too slow thats why I don’t feel better yet.I think i’m doing just fine and i’m going to stay at 50mgs for now and find a new doc. Randee –   In no way am I questioning your personal descision on Zoloft…However, I thought I would share my experience with you…I was at 50 mg for 2 weeks…By then I had few side effects (like you)…However, only my depression seemed better…No effect on anxiety…My doc suggested upping to 100 mg…I said fine since I was having few side effects…Within 5 days my anxiety was markedly improved…I was very glad to have upped my dose…I did have renewed side effects, but these lasted only 2 days (really)…It seemed like going from 50-100 was much easier than 0-50…Anyway, like I said, it is your decision, I just wanted you to know about my experience as it might aid your decision one way or the other… Later and good luck, — Charles Phipps

Hi Randee, Your Pdoc is more than nuts, and I agree, time  for a new one. I am so glad that you stood up for yourself and defended your right to wean on Zoloft  your way, which is the right way. It makes me so angry when I hear doctors with attitudes like yours. I can only imagine how many people have been turned off to anti-depressants because they started at too high a dose, and couldn`t handle it and got off the med. The FDA has issued a new warning on the nonsedating antihistamine Hismanal. See FDA Talk Paper: "T98-5                       Ivy Fleischer Kupec:  301-827-6242 Feb. 9, 1998                Broadcast Media:      301-827-3434                             Consumer Inquiries:   800-532-4440  IMPORTANT NEW SAFETY INFORMATION ABOUT HISMANAL FDA today is warning consumers and health care providers about new safety information regarding the prescription antihistamine Hismanal (astemizole). This warning is being issued in conjunction with the addition of more information in the product label about cardiovascular adverse events, new potentially serious drug interactions and rare reports of anaphylaxis, a potentially life-threatening reaction. The following can be used to answer questions: Hismanal is associated with risks of death due to irregular heart rhythms when taken with certain other drugs and when used at higher than the recommended labeled dose. The new labeling provides information about a specific warning against simultaneously using Hismanal with other types of drugs such as the hypertension drug, Posicor (mibefradil dihydrochloride), and the antibiotics, Biaxin (clarithromycin) and TAO (troleandomycin). The new labeling also provides additional precautions against using Hismanal with a number of other drugs. These include: HIV protease inhibitors such as Crixivan (indinavir), Norvir (ritonavir), Invirase (saquinavir) and Viracept (elfinavir); serotonin reuptake inhibitors such as Prozac (fluoxetine), Luvox (fluvoxamine), Zoloft (sertraline), Serzone (nefazodone) and Paxil (paroxetine); and the antiasthma medication Zyflo (zileuton). Additionally, the label now recommends that Hismanal should not be taken with grapefruit juice. These recommendations are based on the potential of these drugs and grapefruit juice to interfere with the body’s handling and metabolism of Hismanal. The new labeling also emphasizes that patients with liver disorders should not take Hismanal. The new labeling is designed to give health care providers and consumers who use Hismanal the latest available information about these risks. Thanks for the Warning, I already new about the Grapefruit juice. I just started Hismanal a few months ago.  Doesn’t do much for the itchy eyes, but it dries the heck out of my nose and mouth.
