Questions before surgery???
Question:
Here’s where I am — after 10-plus years of sinus problems (which escalated to about 6 infections in the last 8-9 months), my family physician finally sent me to an ENT. ENT sent me for CT scan and allergy testing. CT scans showed cysts (first thought they were polyps, but the report says cysts) filling 2/3 of left maxillary and 1/2 (back half) of the left (largest) sphenoid. There was also thickening in the sphenoid, bottom of left frontal and at the opening (well, where there’s *supposed* to be an opening) to the right maxillary. Ethmoids were mostly clear with just a little cloudiness in some small areas. Dx: chronic sinusitis. Fortunately, I am not in the midst of an acute attack just now. The allergy testing, about which I briefly posted last week, was negative (except for the histamine control). In addition, the IgG/A/M bloodwork that the allergist/immunologist sent me in for has, so far, come up negative — but that’s only the ‘M’ part. The jury is still out on the IgG/A portions — the lab screwed up and forgot to test for these. Had more blood drawn for this early this past week, and I should have the results this week before my visit to the ENT on this coming Thursday to talk about surgery. At the moment I am ‘managing’ the sinus problems with Claritan D (although the ENT says he may have me try a combo decongestant and mucus thinner instead, since the allergy testing was negative and I may not need the antihistamine part, and certainly don’t need the sleepiness it brings). I’m also using Singulair and Nasonex and several OTC things — saline spray, Xlear (sp?), nasal irrigation (still working on the head positioning, Dr. Grossan — thanks for your suggestion!), papaya enzymes, etc., etc. I drink lots of water and actively stay as far from any triggers (to the non-allergenic rhinitis that complicates things) as possible. I do know that when I had to go off the Claritan D before allergy testing that I started with the problems again. Awful headaches — on the right side, and also in back of my eyes and head. And even now, with all this care, I still have headaches at various times. Some days, I still have lots of drainage. I know that the small-to-almost-nonexistant opening into the right maxillary is definitely part of the problem. A couple of weeks ago, the whole side of my face was throbbing — finally I heard a long, drawn-out "sqeeeeeeeeak" (loud — like someone had stepped on a dog toy or something — even my husband heard it) and the headache disappeared. So I know at least a portion of this is the dreaded vacuum effect. Unfortunately, all my manipulations cannot quarantee that I can open it at any given time — even after all my various methods are tried. Once the headache starts, I’m out for the count unless I get lucky. Sometimes it lasts for days (even through the night). I am so tired (fatigue has been my middle name for years), and I am about 95-percent decided on surgery sometime in early October. I’ve done a lot of reading here and other places on the risks and rates of recovery, and I’ve read tons of post-surgery stories (both good and bad) and have spoken with people who have had FESS (and had good outcomes). I’m putting together a list of questions for the ENT appointment. I’ll be taking my husband along so that he can ask questions and listen to the answers as well. I believe that the ENT wants to avoid too much work in the sphenoid since the nerves or arteries or whatever are so close to some of the thickened portion, but at this point he believes that getting rid of the cyst is possible. He would also remove the cyst in the left maxillary and enlarge the opening to the right. Fortunately I have a beautifully straight septum which requires no work whatsoever. Thank heaven for small favors. Is there anything that, in the experience of those here, is important to ask at this meeting to discuss surgery or that you should have asked but didn’t? Also, any suggestions on building up your body/health before surgery? I start back to work in September after five years home with my son — but fortunately I’ll be working with my husband, and he’s pretty lenient (if he knows what’s good for him) about the time I need to take off to recover. I sure would appreciate any input on this.

I think it is important to evaluate the experience and reputation of the otolaryngologist. I would look for someone whosr primary specialty has been sinus surgery for many years. A University may be one place to look.

Hi All, My Doctor has treated my cough with hydrocodine when it was determined to be from bronchitis and not prouducing any mucous. The continuous coughing without bringing anything up caused me a lot of pain. Just another side of the story. Mike C.

Don’t know a much technically, but I occasionally use Tussey-Organdin, and it seems to help greatly.

Over here on the East Coast the pollen count is still quite low, oh to be in lotus land like you. On the more serious side, we used cough medicine (Triaminic) for my young son’s asthma early in his condition and it helped. I tried it recently when his asthma coughing was much worse and it didn’t even touch it, our **G.P.’s** question when asked was whether it was an antihistamine or not, since antihistamine would impact the underlying problem whether in cough medicine form or separate. It seems the proper cough medicine will give some relief which is good by itself, but, as noted in Wing Lee’s posting, if it’s only suppressing the cough you still have the mucous build up in the lungs. From that point my wife (who is also asthmatic) and I think that **regular** use of cough suppressant in itself is probably not getting to the route cause. For my son I think he wasn’t moving the mucous, therefore it built up, and finally his coughing kicked in just as hard, but possibly with worse underlying lung function. I believe this type of problem is the reason for the warning for asthmatics on cough medicine bottles (at least the ones I’ve read), i.e.: a) if needed routinely you need may need to consider other management options, and b) the **possibility** of masking more serious attacks. Hi I’ve used Benylin and Delsym for my coughs with no problem along with taking my Pulmicort and Ventolin.I would use it only at night to get some uninterrupted sleep. It is not recommended that asthmatics use cough syrups because they will mask an uncontrolled attack.I would check with your doc with doubling your dose of Pulmicort to get the asthma under control then tapering down to a maintenance dose.I find that the Pulmicort usually kicks in within 2 days, sometimes longer if I’ve let it go a little too long.If it doesn’t kick in and the cough worsens you may be on your way to needing some extra meds. Hi, My neurologist (I’ve been diagnosed with a movement disorder — most likely a form of spinocerebellar ataxia) prescribed 10mg Lexapro to combat associated feelings of depression and anxiety. Unfortunately, it has made my essential tremor much worse. Has anyone had a similar problem? Any ideas for combatting this side effect? Thanks, Matt
SSRI’s can exacerbate movement disorders and even cause some (in rare cases). I want to thank all of you for your responses. I talked to my neurologist and she switched me to Paxil. My tremor is much better (very close to its pre Lexapro level). Start at Lexapro 5 mg/day for awhile before you go up to 10 mg/day. Add a beta blocker such as Inderal or Tenormin for the essential tremor. Chip — The charter is available at: http://readystump.algebra.com/~asapm

as chip said a beta blocker. Or you could also use primedone or valium, that has helped me with my tremors. the primedone, i think the brand name is mysoline is good but it was rather strong for me. it stopped the tremors to 0 but all i would do is sleep. brian s.

Thanks for your response. I didn’t get a hand tremor until a month ago. The gyno doctors wanted to prescribe tranquil. because they felt the headaches, nausea and intermittent fevers were not menopause related and I looked depressed. I explained they would look depressed too if they sat where I was sitting with a headache, nausea and a 99+ fever. I couldn’t possibly be cheerful. No one tested me for depression. I’m waitng for an MRI and neuro consult. Although I never heard of hand tremors associated with menopause I was hoping maybe it was a rare symptom rather than the alternative diagnosis of brain inflammation, brain tumor, Parkinsons. Dale

Hi Dale and welcome to a.s.m. For what it’s worth, I have hand tremors under two circumstances, neither of which is, I believe, directly related to menopause. In 1998 I had a neuropraxia of my right hand. I thought I had had a stroke and went to the emergency room. Believe it or not, my doctor advised acupuncture (which I was getting for a shoulder injury) and it eventually went away by itself. Affected my handwriting something awful for a while. Now I get hand tremors from pain. I have recently posted under other threads about dealing with unwanted facial hair and, respectively, electrolysis and most recently (today, in fact) laser hair removal. The pain causes hand tremors. I think it is wise of you, though, to have a checkup to see if there is another reason for them. I have had headaches for many years, but nausea and fevers (unless you are referring to hot flashes, which, at least in me, do not cause actual measurable fever) is a good thing. I understand your frustration as I went through many years of ‘mis-diagnosis’ before I got an answer to all my symptoms. The best advice I can give is for you to ask your neurologist for a brain MRI scan, it’s a painless non-invasive procedure. It linked to http://rxlist.com and I noted the connection between Effexor and blood pressure. Though the data played down the impact of Effexor, it did indicate that Effexor did raise blood pressure in proportion to dose. I just had my blood pressure taken after 9 months of Effexor XR 150mg. The first systolic reading was 170 and the second was 160. The nurse was worried that I was about to have a stroke! I stopped taking it for several days, today my reading was 145/90 (still too high but not stroke-level). I am going to go for a week without it and see what my blood pressure drops to. Although I am somewhat overweight and my diet is not too good, I am convinced that the Effexor is the chief cause of my high blood pressure. Just 2 years ago it was only 125 in a doctor’s office.

Hi Stan and Teilhard I stopped taking one type of medication because my husband said it didn’t make me better. he said I was better on the previous type. So i stopped for 3 days to switchover. but i didn’t cut down first. that’s where i made the mistake. i’m already in therapy and have been for a year. i don’t know if it helps. i’ve been up and down throughout the year and this time i’m back to where i was when i started the therapy. no perhaps this time i’m worse. this time i want to die. i haven’t felt this bad before. i’ve told my son that i’m very sick and may not get better, so he’s prepared. i can only see this as the way to free the pain. otherwise it will always be with me here. sue

Welcome to ASD, Sue. The only think I can recommend is looking for professional help. Seems you have seen a pdoc by your reference to medications. Why did you stop taking them? I hope you didn’t do it without supervision, that might be the cause you are feeling bad now. The other thing is therapy, but only you know whether you need it. Hi Stan and Teilhard I stopped taking one type of medication because my husband said it didn’t make me better. he said I was better on the previous type. So i stopped for 3 days to switchover. but i didn’t cut down first. that’s where i made the mistake. i’m already in therapy and have been for a year. i don’t know if it helps. i’ve been up and down throughout the year and this time i’m back to where i was when i started the therapy. no perhaps this time i’m worse. this time i want to die. i haven’t felt this bad before. i’ve told my son that i’m very sick and may not get better, so he’s prepared. i can only see this as the way to free the pain. otherwise it will always be with me here. sue

Hi SunKitten, are you finding the Zoloft works for you? my son was on zoloft but we have weaned him off it. It’s been 6 weeks now. he goes back to school this week, so this will be the big test to see if he’s going to cope without medication. He’s only 7, so we don’t want him on medication if he can do without it. Sue

You made your mistake when you let your husband make your medication decisions for you. Is your husband also your medical doctor?
no, but he is right. i can see that now. i have thought back over the last few months while taking a new medication, and i really have been worse than previously. Because if he isn’t, then the only person who should be telling you what medication is helping you, is YOU. I get the feeling that your husband is also the one who taught you to refer to your depression as "madness", as you called it when in your first post you asked when does the madness stop. Depression is not madness, it’s a disease that mingles physiological problems with psychological problems. Most people respond best to a mix of the *right* medication and the *right* therapy. But it has the be the meds and therapist that are right for YOU.
I don’t know if I’ve found the right therapist. I’ve been to so many before, and this is the first one i’ve stuck with. Usually i stop going after a couple of sessions. Have you talked with your medical doctor at all about changing your medication? Have you discussed the different types of medication that are available in Australia, and decided with the advice of your doctor what you should be taking?
yes, my psychiatrist has been in contact with me over the weekend. I’ve tried many different tablets. sometimes they work for awhile but then dont seem as effective. Which meds are you taking, which meds were you taking, and never mind what your husband says makes you better, how did you feel while you were taking them?
I’ve just gone back onto Aropax (Paroxetine). I was on Efexor-xr. Folks in asd will gladly give you all the support and understanding (and information about our own depression experiences) that we can. Gosh. How long had you been taking Effexor? It is one of the anti-depressants which must be quit with outmost care, AND supervision. It takes about three weeks (or longer) to make you feel better, and I do not know how you should quit, because I am still on it. Best of luck with the new Message board, but I won’t be participating there because I don’t like the idea of moderated groups/boards. I have lived with an ileostomy long enough not to be bothered by idiots who don’t know what they are talking about or by trolls tossing a ‘bomb’ into a discussion group. I can understand that some people are more sensitive to these things, but IMHO it is better to say to yourself "What a JERK that person is", delete the message and give it the attention it deserves which is ….. none. I can’t understand why someone would want to cause negative feelings in people, but I admit that some mentally challanged people do want to do that. If you respond to them in any way they get some kind of sick kick. I don’t want to give them that pleasure. Rob PS If you have to reply to a jerk (sometimes it helps to vent your anger) you can do it with an email to them and save the rest of your newsgroup from having to see the subject line over and over. Just a suggestion. Everyone is entitled to do what they need to do to let off the steam an idiot’s message can cause. PPS Regarding SPAM in newsgroups. That is so excellent, every step in the right direction is a huge step forward! ok…went to see my ob/gyn yesterday for a checkup…and i finally broke down and told him i was suffering with depression…i was so embarrassed…started crying and everything…but he gave me some sample of this drug called Effexor…(spelled right?) i haven’t started taking it yet, guess i’m a little nervous of the side effects, if it has any. and he also gave me a couple of 800 numbers to call to see about therapy…too nervous to call them yet. but just wanted everyone to know, i’m on my way to getting "there" hopefully will be "happy" again… Toni

Thanks. Staying silent seems safer, but I know talking and stepping over the other boundaries that I think keep me safe is the only way for me to make progress. but he doesn’t think I’m doing enough to fight my depression, and that I’m just giving in and staying in bed too much. <nod, nod I know this one.
It’s hard for people, even those who, like Leigh, have battled some depression themselves, to see things from my point of view. When some days just getting out of bed is a great achievement, or even just staying alive, it seems to me like I’m making a huge effort. But other people can’t read my mind, and they don’t know how hard some days can be, so to them, it seems like I’m doing nothing. , and my moods can change so rapidly Mine too.
This is new for me, since I’m not bipolar. All I know is that one day, or even one hour, I can feel fairly good, and the next I’m crying or just unable to function at all. It’s taking a bit of getting used to. I just can’t sleep properly. I just want to feel rested, and to wake up and not still feel tired. It would be great not to wake up heaps of times during the night. I’m back taking valium again, OK, NOW you’re really starting to sound like me but I went on Xanax.

Thanks Mary Beth. I’m glad you’ve found a pill that helps you sleep, at least at the moment anyway. Luckily, I haven’t had many medication induced dreams (in fact, I rarely dream at all) so I really hope those go away quickly. I hope opening up a bit and posting again helps me, because nothing else seems to be working anymore. Also, thanks for responding to my post. You don’t usually respond to things I write, and I was starting to wonder if I’d offended you or something. I couldn’t think of how I might have done that, but I felt like I was being ignored. I’m glad I’ve been proven wrong, and I can assume those thoughts were my own paranoia sneaking in again. I would have said that via e-mail, rather than posting it to the group, but my proxy server is screwed up at the moment, and I can’t access my web based mail, only the account and newsgroups I have set up through Outlook Express. CAREFUL WITH THE VALIUM. It is a muscle relaxant. And your Heart is a MUSCLE. I do hope you find something to help you get some sleep. Have you tried Ambien or Deseryl? I flip and flop like a fish out of water at times when i try to sleep at night, i have been meaning to go to a sleep clinic. If you constantly wake up, you will never get into your REM stage of sleep and you will feel tired all of the time. I hope that you are doing better. Sincerely, Trish

It sure sounds like you’ve been through an unbelievable amount of shit. I can’t even imagine.
Yep, an "unbelievable amount of shit" sums it up pretty well, although there are others here who have been through worse. I’ve been diagnosed for about 14 months now, and on meds for about 8 months, yet nothing has made any difference. I’m now failing out of uni because I can’t function properly, and I’m just trying to hang in there until my psychiatrist finds a medication that works for me. He does believe I will respond to an SSRI, but since there’s a hell of a lot of them it could take a while to find the right one. I have depression and have been "lucky" – I found a – Hide quoted text — Show quoted text – great shrink many years ago who knew what she was doing about medication and put me on Parnate, which I have been taking for almost 10 years. I also have seen her on a regular basis for psychotherapy. It sound horrible that you had to experience these kind of side effects on your medication. A competent doctor should work with you to find the best medicine for you which does not have those kind of effects. There are so many medications available now – for me, I only got real help when I went to a shrink who was trained in psychopharmocology. It was a great combination – she knew everything about the medicines and was a great psychotherapist. There are a lot of people out there like that. Maybe you could find a better shrink/therapist who would work more closely with you to get the right meds which don’t fuck up your body. Good luck.
I’m not considering changing therapists or psychiatrists yet, because I have a great relationship with my therp, and my pdoc’s specialty is abuse and trauma survivors, which is what I need, and he is nice. I will admit that the side-effects are a total pain in the arse, but hopefully I will find a good med soon which will work, so I don’t have to keep getting used to new meds and adjusting to new effects. Thanks for caring, and I’ll definitely keep what you’ve said about changing therps or pdocs if I don’t start making some progress soon. Before then, I had days of vomiting, and one really scary day when I couldn’t stand up or walk without feeling like I’d pass out, and my hands and feet going numb. I ended up at the hospital for that one, but there wasn’t anything they could do. I just had to adjust. Leigh and I are still together, but our relationship has been weakened recently by lots of fights, all caused by my moods. He’s trying to be there for me, and understand how I’m feeling, but he doesn’t think I’m doing enough to fight my depression, and that I’m just giving in and staying in bed too much. I tell him that I can’t leave my little room to stay at his place overnight, like I do every Tuesday night, and he gets angry. I just need some space sometimes, and my moods can change so rapidly I just know I need to be alone sometimes because I don’t want to say something and hurt him. I love him too much. I haven’t seen my therp or pdoc in weeks. I’ve spoken to both of them briefly on the phone, but that’s it. I was genuinely sick last Monday, so I cancelled my appointment, and spoke to my therp on the phone instead. This Monday though, I wasn’t too sick to go. I just couldn’t leave here, so I rang up and said I was still sick. I only spoke to the receptionist. I know I should get out of here and go see someone, but I don’t know if I can. I’m so tired, and all I want to do is sleep. Getting dressed and walking to the counselling centre seems like too much. I just can’t sleep properly. I just want to feel rested, and to wake up and not still feel tired. It would be great not to wake up heaps of times during the night. I’m back taking valium again, and even that isn’t working. The last two nights, I’ve taken more than I’m supposed to, hoping that an extra pill or two will be enough to let me sleep. It still hasn’t worked. I take the maximum doseage when I go to bed, and still wake up within a couple of hours. So I take an extra pill, hoping it will help me get back to sleep and stay asleep. But I still wake up again and again. After a while I take one more, just hoping I’ll be able to sleep, but nothing works. And yes, I do realise how scary that sounds, but I’m not going to take too many. Well, I guess that’s all the ranting I have to do at the moment. Sorry for not posting lately. There’s been a lot I wanted to say, but I just haven’t been able to come out of my shell. Figures the time I do come out and post again, it’s for totally selfish reasons. Hugs to everyone. Thanks. I didn’t think anyone had noticed I was gone. I was hoping someone would care and post an MIA or something, but I needed to talk so I decided that my pride isn’t that important after all, and I should do what’s right for me, which is posting when I need to. Thanks. I didn’t think people would notice that I was gone. I know how you feel about tiredness… I’m on 225mg daily of effexor, and i constantly feel very tired….. I sleep ok, but i wake at 7am every day

I understand. Last year, I had problems falling asleep. Now, I can fall asleep fairly quickly, but I just can’t stay asleep. It’s become normal for me to wake up every 2 or 3 hours for no reason at all. have you tried herbal sleeping pills??? They may mess up with your meds…. Not getting enough sleep is annoying….
My boyfriend gave me some herbal pills called Valerian which sometimes work, and they don’t seem to interfere with my meds, but the instructions say take 3-6 tablets at night to improve sleep, and it takes the full 6 to have any effect at all for me, so I’m going through the bottle very quickly, and they’re expensive. Unfortunatly I’m on a limited budget, so that has to be a consideration. Have you mentioned your sleeping problems to your GP/therapist???
Heaps of times. They keep telling me the antidepressants or the valium will work, but since it’s becoming more and more obvious that they aren’t, hopefully they’ll decide to try something else. Don’t worry for posting for selfish reasons…. you have helped us loads of times in the past, so it’s time that we helped you….

CAREFUL WITH THE VALIUM. It is a muscle relaxant. And your Heart is a MUSCLE. I do hope you find something to help you get some sleep. Have you tried Ambien or Deseryl? I flip and flop like a fish out of water at times when i try to sleep at night, i have been meaning to go to a sleep clinic. If you constantly wake up, you will never get into your REM stage of sleep and you will feel tired all of the time. I hope that you are doing better. Sincerely, Trish
Thanks Trish. I know it was stupid to take that much Valium. The only good thing was that I spaced the pills out through the night, rather than taking double my maximum dose all at once. I’m just willing to try anything if it means I’ll get a good nights sleep, and not constantly feel exhausted. I had no other intentions. I’m feeling a little bit better at the moment, but I hope stress doesn’t kick in too badly with my exams approaching, because I know how much that effects me. I’m just hanging in there and hoping that my pdoc will have some new suggestions when I see him next week. Again, thanks for your concern. I know it was a mistake, and I won’t experiment with my valium doseage again. Kylie.

It sure sounds like you’ve been through an unbelievable amount of shit. I can’t even imagine. I have depression and have been "lucky" – I found a great shrink many years ago who knew what she was doing about medication and put me on Parnate, which I have been taking for almost 10 years. I also have seen her on a regular basis for psychotherapy. It sound horrible that you had to experience these kind of side effects on your medication. A competent doctor should work with you to find the best medicine for you which does not have those kind of effects. There are so many medications available now – for me, I only got real help when I went to a shrink who was trained in psychopharmocology. It was a great combination – she knew everything about the medicines and was a great psychotherapist. There are a lot of people out there like that. Maybe you could find a better shrink/therapist who would work more closely with you to get the right meds which don’t fuck up your body. Good luck.

Hi sultan, My weight has remained about the same. I do have a better appetite on Zoloft cos I really didn’t eat much (mainly diet coke and cigs) at all cos I was always anxious or preoccupied. I think the reason for not gaining is that I am physically doing more since I feel more normal on Zoloft. Paxil (which I first tried) had me craving odd things which turned into a 5 lb gain. This was a mixed blessing as there is a restaurant 2 houses down from me with an all you can eat 24/hr day buffet. The appetite had me getting out the house behind these cravings which was good….the forced to leave the house part, but I always got it to go (cancels out the all you can eat benefit) cos I couldn’t stay. With Zoloft, I still don’t eat as much as anyone I know, but I do eat more regularly than every 2 days!! One day I ate 4 times (a record), instead of 1 time,but the Paxil made me feel sluggish and then with all that buffet food in me, I wasn’t progressing just gaining weight and/or sleeping……still saw no sign of life!. YMMV, cos this is definately different than my experience. I usually can eat about an hour after a benzo. Guess it takes that to calm me down enough to get into it. Miriam

Yes I have gained 25lbs in two months. I have never had a weight problem. I can do nothing to stop the pound a day I see every morning. It is really beginning to concern me as my body doesn’t seem to respond to any attempts at weight loss. I am taking it for anxiety but if it’s going to work for me it will be at a higher dose because so far, I still require the xanax daily to stay normal. I do not want to continue with this med if I am going to continue to gain–surely there is another med out there not destined to do the same thing but will still help with the anxiety. I am interested in hearing from any and all of the responses you have received to this post.
