ONe doc told me to put my inhalers in my mouth when i use them…..another told me not…..i would appreciate if others would share what their docs said!!

If you do not have a spacer, then out. place the mouthpiece about two to three fingers away from the mouth and proceed. The idea is to slow the delivery of the aerosol so as not to just coat the inside of the mouth… basically what a straight spacer does. With less medication coating the mouth, the more you’re able to take in.

Inhalers come with patient instructions so the first thing to do is read them. Different kinds of inhalers may be used differently. Probably the most common inhaler type at the moment is the MDI or metered-dose inhaler. There is more than one correct technique; theoretically spraying a short distance outside the mouth results in better aersolization, but aim is critical and its hard to get the spray going in the right direction. MDI instructions usually show the inhaler being inserted in the mouth since its easier to aim. The best way with an MDI is to use a spacer, like an AeroChamber–this results in good aersolization with minimum side effects due to overspray. The breath-actuated inhalers, like the Autohaler and DPI (dry powder inhaler), like the turbuhaler, require the mouthpace be inserted in the mouth to get proper suction. I am using Serevent and Flovent 220 mcg, 2 puffs of each in the morning and then again in the evening.  I use the InspirEase spacer to take my medication.  Both medications state they contain enough medication for 120 puffs, which for me is a months worth of medication.  However, I have noticed that after a month instead of the cannisters being almost empty they are still half full.  I put them in water to measure them.  I am wondering why this is.  Could it be that the InspirEase doesn’t extract enough of the medicatiion from the cannister with each puff? All the spacers I’ve ever used simply had an opening on one end for the inhaler (where the mouth would usually be if not using a spacer) and an opening for me to breathe from. If that’s the case with InspirEase, then it wouldn’t make any difference in the amount of medicine that comes out when you press the canister down. I had actually read on some inhalers (might have been serevent, but I don’t remember) that that water measuring trick wasn’t very accurate and the best thing to do was keep track of how much you’re using. Also, is it possible that you have forgot to take every dose in a given month?

Exactly..the only difference is that it has accordian -like folds and collapses as you take the meds…;

The canisters are otherwise with propellant in order to ensure that the first actuation with have the same pressure as the 120th actuation.  The problem is that after 120 doses all you will be getting is propellant with no medication. No electrons were harmed in the posting of this message.

Melatonin and asthma meds.

I don’t know much about what melatonin would do, or have any authority in the medical area at all. This is widely hyped as a ‘natural’ sleep aid. A good write up on it can be found at:

http://www.hcrc.org/cgi-bin/mfs.cgi/20/web/u7/www11417/www/contrib/ac… cles/melaton.html I use melatonin once in a while.  If I’m having trouble sleeping I much prefer a combination of valerian, skullcap and passion flower.  You can’t take valerian every night though, so I only add that if I really need it or at a maximum of 3 nights a week.  You can take everyday the first week you use it or if you do not use it often (IOW if a break of several weeks occurs between usages) but don’t take it every day for longer than a week. Valerian is also a natural muscle relaxant.  If it were me, I’d use it every night for 7 nights and then 3 times a week thereafter if I had chronic muscle cramps.  It works quite well and you should find that you don’t actually need it every night after the first go.   Skullcap and passionflower can be used more often.  Skullcap is a lovely soothing herb that helps the nervous system as well as helping with sleep. I prefer to use melatonin only when traveling internationally or across several time zones.   Melatonin is a hormone produced by the body but the amount the body produces decreases with age. I have not found it to react with my asthma medications which include Flovent, Serevent, Singulair and Albuterol.   Loki

Ellis; thank you for your reply, I looked up some of the reactions of drugs I took, CECLOR,ASMACORT, PREDNISONE,AEROBID,VANCANASE, MEDROL, DELTASONE. HISMANOL, AND INTAL(THIS ONE JUST LOST ANY HELP AFTER OVER A YEAR ON IT.  hives, 43#  rapid gain wt. headaches and abdominal cramps, and fluid accumclations in large amounts.  the preventil-albuterol and the intal, accolate,  was good, the singular switch seemed o.k.     I also have tried over and over to use the flo-meter and can never get it over 140 course I havent blown up a bloon since I was a kid.     Because this doctor  is good enough to see me after I challenged the other one I am afraid to make waves     again thanks for the concern i will read the other comments as they help my attitude, glad i was older before I got this problem, really can’t complain as I had a good life. For two years I took melatonin every day , I have asthma and was taking abuterol,x2 puffs, 4times a day and at night taking singular before bed. about three months ago I had over twenty attacts of sever muscular cramps in my rt. upper quadrant, so severe I was in tears.  My dr. put me on LIVBID and it corrected  the pain quickly, the day I started livbid i quit the melatonin.(took it to help sleep and my hair grew longer than ever in my whole life)  Now if I go off the livbid my pain returns in my right Quadrant  This pain has been an off again on again problem with me four or more years before I was diagnosed with asthma, say about 14 years and I have been through the standard upper, lower, etc  type exaimes.

Re: Pain in upper right quadrant; could be liver problems. There is a warning on Accolate (similar to Singulair) to notify doctor if this happens; liver function tests may be needed. Re: Singulair; this is a new drug that helps about 2/3 astmatics who take it. If it doesn’t help it shouldn’t be continued. Sometimes a smaller dose is prescribed; the 5 mg (child’s dose) instead of 10 mg adult dose. Older adults metabolize the drug more slowly. Re: albuterol 2 pf x4. New asthma guidelines indicate if you need more than 1 pf/day, inhaled steroids should be added or increased. You didn’t mention taking any inhaled steroids (Vanceril, Azmacort, Pulmicort, Flovent, Aerobid). These are the main treatment drugs for asthma. Another drug to try in your case is Atrovent inhaler. It’s also available as Combivent, a combination of Atrovent and albuterol. Re: LIVBID  I think you must have the name wrong. It isn’t listed in drug books and a web search came up with nothing. By understanding asthma better, you can tell whether your doctor is treating it effectively. If you aren’t on a steroid inhaler, I would want to know why not. (Pulmicort or Flovent best) You might benefit from a 2nd opinion from a major asthma clinic or teaching hospital. I have heard of a saline spray for blocked throat in glandular fever. Also the lungs are high in sodium compared to other body tissues. Cramp can be a result of too little salt. Have you changed your diet apart from melatonin? Perhaps salt helps to slow the heart, a little, too, maybe if it is with some potassiium chloride, too, and helps sleep. That would be if your kidneys are not hanging on to salt because of lack of the hormone aldosterone. Or perhaps if you take diuretic. Sometimes you are told you should not reduce salt intake when on certain diuretics. The sodium/potassium balance deserves care, also when experimenting. I am a 23 year old who has had excercise induced asthma since I was 16.  Until I was 22, I was able to control my symptoms with Proventil. In the past year, I developed horrible asthma symptoms on a regular basis, not just from excercise.  While I have been able to control the symptoms with Pulmicort and Serevent during regular activity, I am still unable to excercise easily.  It take me weeks to build up the ability to jog on a treadmill for 10 minutes.  I have always been an active person and have become frustrated and angered by my inability to excercise.

Ask your doctor about FloVent.  I consider it a miracle drug since my asthma and COPD are slowly improving ever since I started taking it. If anyone has any suggestions on medications and/or other techniques that help them with their excercise induced asthma, please contact me by email at Monitoring you lung condition with a peak flow meter can let you know where you are at: an Action Plan can be used to adjust asthma drugs per peak flow readings. Before vigorous exercise you should be near the top of the Green Zone. Monitor the smog level and pollen levels (if allergic) and exercise when levels are low; usually in the morning or late evening. I would suggest you speak to your pulmonologist about one of the leukotriene inhibitors (ACCOLATE, ZYFLO or my preference SINGULAIR).  All of these have been beneficial to various degrees in exercise induced asthma. Scooby RCP, EMT-P

Hi I’ve found the Buteyko breathing technique very useful when exercising, I now no longer need to use Ventolin before exercise. Give it a try. Buteyko is a breathing technique. But unlike many other breathing methods Buteyko’s aim is to breath less. Buteyko has improved my asthma, reduced my Ventolin to zero and my steroids by half. I would recommend all asthmatics to try it to see if it works for them. However a word of warning, some have felt so good after using it for a few months they have thrown their medication away. You should continue to carry your medication even though your asthma may be in remission. Buteyko is a great method to help reduce asthma – it is not a miracle cure. For a good introducing and more information on Buteyko try http://www.wt.com.au/~pkolb/buteyko.htm. It is best viewed by using the option to download the whole site and then read at leisure. It could be the propellant you are having a problem with or the medication myself.  I had problems with aerobid inhaler, it made me feel worse.  My doctor switched me to azmacort and I am free sailing now.

A few years ago, my pulmonologist put me on Becloforte. After a few months, trying to inhale it was making me caugh with the result that I was not inhaling it at all or almost. He then switched me to Pulmicort and I had no problem. I had maybe, he said, become allergic to the gas propellant… Yves Dussault

Severe reaction to Buspar???

HI All,    I have suffered on and off for years with depression and anxiety.  I have been on Prozac, Zoloft, Effexor SR and Serzone.  I have been off them for over 2 months (serzone was my last).  Well the anxiety is still there (no depression to speak of) so my Dr. prescribed Buspar today.  I took  5 mgs.  and within 1.5-2 hours I was dizzy, had a bad headache, was sick to my stomach, sweating uncontrollably and salivating like a dog.  My speach was also slightly affected.  Is this a usual reaction to Buspar at such a low level?  I never had these reactions when taking any of the above listed drugs.  I threw-up for over an hour and then felt fine.  I want to control my anxiety but cant if it will always cause this strong of a reaction.  I came off effexor cold turkey (dont try it) and did not feel this bad.  Any input would be greatly appreciated.  The rains have began once again here in Houston.  Tropical Storm Allison wants to take up permanent residence!  Thanks.  Randall in Houston

I’ve been on 60 mg. of Buspar a9the maximum dosage allowed) for 8 weeks. Anxiety is way down and the only side effect I get is some tingling in my hands, I think. M

I always say time and time again that these cocksucking shrinks prescribe WAY rto hign a dose to start out. Many people for whatever reason cannot tolerate these doses prescribed to start. My advise to you is to start out at 1/5 the dose your doctor prescribed.

thanks to all your responses.  should i cut the 5 mg in half and really slowly ramp up?  i would think that since i reacted so severly to 5 mgs. of buspar that started out on Effexor or Zoloft would have really upset me too.  thanks…randall

Hi, I’ve been sufferig from epilepsy for about 12 years now, trying all sorts of medications and combinations without getting any noticable improvement. A year ago I started a new combination that improved my condition greatly, Sodium Valperate 700mg 3 times a day & Lamictal 75mg twice a day. unfortunately it brought with it some very aggrevating sideaffects. Several months after starting that combination,my right forefinger started trembling uncontrolably, later my pinky started the same kind of tremble and at the moment I’m starting to develop the same kind of tremble in my left hand as well. This is a very light tremble (not like "Parkinson"), steel it is very disturbing. For example, it forces me to hold my forefinger with my thumb if I don’t use it while typing to prevent it from disturbing my conentartion (this is only one example). Does anybody have any experiance with the use of this combination ? If you do, let me know. Thanks, B.G

Your best bet is to have it checked out next time you see your doctor.  It may be side effects from your meds that could be easily corrected.

Hi BG, My daughter Harley used to be on that combination and i`m afraid to say it made her very very poorly ;-( She used to get "tremors" with it…in fact she got so bad that her whole body shook. But she obviously had a bad reaction to it. Could it be breakthrough sz`s? She also tremors from the ep. Have you ever tried Topiramate?…When Harley had to be weaned off the Lamotrogine,she was put onto that. Although she still doesn`t have full control,it has been the best combo she has had,and at the moment seems to be doing really well(for her!). My youngest son Marshall has just started on Lamotrogine with Epilim too,so please wish me(him!) luck! I hope you get some answers,please let us all know how you get on. You may want to be checked out by your doctor to see if it is side effects of medication.  You may also want him/her to check and see if it is the neurological movement disorder called essential tremor.  I have this and it makes me shake uncontrollably like you are talking about.  Hope you get the help you need. Hi, I see that you’ve already got some good recommendations already. It helps to know that you’re not alone, doesn’t it? My nieces has epilepsy and I have narcolepsy. I was put on a drug many years ago which I thought was an answer to prayer. It was called NARDIL. I have heard that it’s been around for a long time and not used too much anymore because of it’s diet constraints. If one doesn’t follow the diet restrictions instructed while taking the drug it could cause stroke/heart attack at any age. Anyway, I felt great on it initially,….was awake and had energy during the day. THEN the weird facial palsy stuff started happening. First gradually, just at night a couple of times and then more often and happening during the day. I was scared and didn’t think about relating side-effects to the drug at first. Then another side-effect was that I *couldn’t* get more sleep than 4 hours at a time and my body became more and more exhausted. The drug wasn’t "letting" my brain have normal sleep anymore in reverse. Narcolepsy without medication is when you fall asleep ALL the time. Needless to say, I reported to my doctor and was taken off the drug. She told me that the side-effects I had did happen to some people when taking Nardil. Additionally, once I was given *Phenothyiasine* (sp?) when I was a teenager for "dry heaves" when I was extremely ill with the flu. Gross,…but I kept retching even when I didn’t have any more to throw up. (sorry for description) By end of day two on it, it was throughing me into seizures. So unfortunately, what you’re experiencing could be directly related to your medication, but as the others have urged,….it’s best to go to your doctor about it. <Posted and Mailed One thing that my therp threw at me that was hard to accept (okay…still is), but I know is true, is that we cannot control the way we feel, only how we act because of our feelings. Yeah, it’s kind of warped sounding in a way, but it is true. For instance, I can’t stop myself from feeling I’m a completely useless turd, BUT, hopefully I can stop acting like a completely useless turd until I finally (maybe) feel differently.

This is similar to what I call "acting you way into a feeling". It is far easier said than done — particularly when you’re very depressed. But for example lets say that you are mildly depressed and don’t want to go to a party that otherwise you would want to go to. But you are going to make the effort to go anyway in spite of how you feel. You decide that you are going to act like you are having a great time. So you put on your best party grin and start mingling and talking. Pretty soon you find that you are no longer acting a role — but you are really enjoying yourself. This technique works better with situational depression than with bipolar depression. It’s much harder to fool neurotransmitters!

permanently in the ether: I wasn’t aware that Buspar was so short acting. I take 30 mg at bedtime, it used to be 15mg am and pm, but the am dose was definitely making me sluggish. Of course, it could have been the paxil that was knocking me out. I have no idea how effective the Buspar is; because I take so damned many drugs, I never know what is what.

The drowsiness you describe does sound like what happened to my MIL her first few weeks on buspar–then she adjusted–but she does take a nap after lunch and sometimes tries to take on after breakfast. Buspar has made a world of difference for her.  Before buspar, she refused to go to any activities–and kept asking ppl to take her home. After a couple weeks on buspar–she turned around. These drugs have so many different effects on different ppl.  When I was in college I took a pharmacology class that was a survey of all drugs, from caffeine to heroin to thorazine.  One of the first things covered was the LD 50 and the ED 50. LD 50 is the lethal dose for 50 ppl out of 100.  That means the lethal dose stated on your insert will kill 50 out of 100 ppl, of the  50 ppl left, some will die at a lower dose and some will not die at the LD 50 level.  I dont like those odds. ED 50 is the effective dose for 50 out of 100 ppl.  So if a dr prescribes a med based on the medical information provided, you only have a 50/50 chance of the drug even being an effective dose for what you are trying to change.  You also have a chance (if you are one of the 50 ppl that the LD 50 doesnt work for) of the drug being either too strong or too weak for the benefit you are after. I find this whole medical crapola to really be one big game of Russian Roulette.  JMAO.    Yes, I have been having a bad time, but it is getting better. Of course, I have taken a fatalist attitude toward my problems. There is nothing I can do about it, so if they shut off my cable, internet, and telephone; oh well. I don’t think they can shut off my electric because it is my heat and a/c, and with my asthma I must have a/c. Of course, many times in the past, I have discovered the hard way that they can do things to me that I thought they couldn’t.

Please contact the power company if you are not already on lifeline service.  If you are not on lifeline service–they can cut you off. Even my kid got lifeline and he is fairly normal. :-)   Also, I thought cable companies were  required to provide a basic economy package for ppl with low incomes?  Maybe it is a california thing–but here it is the law. hi…me again! for what it is worth, i find it interesting that you get sluggish from buspar…when they had me on that stuff i felt like i had water flowing through my ears constantly, and i was always wired…i never slept. i went on xanax and zoloft after that and even though the zoloft made me that way for around a week, i got over it. but buspar never agreed with my system.(made me want to call the phone company to thank them for billing me…ugh.)…bruce

And now I’m wondering–if you take buspar at bedtime–it wears off in the middle of the night–sending you into the depression during the day that you have?  I may be totally off base–but this is a thought that occured to me. And I have been given Lorazepam (ativan) for anxiety and maybe buspar might be a more controllable choice for me.  I have never used up a script–they go out of date on me when the bottle has been half used.

Hey Ralph you think the Buspar actually works? I was kin a thinking of blowing off refilling mine this month. The freaking prescription is $80 to refill…….anyway. Maybe the therapist will help? Dear Robert,         I don’t know about the buspar, myself. It seems to help me sleep when I have a lot of anxiety. I also seem to forget to take it, a lot. I never forget to take my Prozac. Boy, am I grateful that my Medicare HMO has a RX plan. Without it, my meds would be over $700.00 per month. I think what I am trying to say is that Buspar helps when I am really agitated/anxious and can’t sleep; but I question the usefulness of it day in and day out.         Thank you for your kind words. Yes, you are right, I am not totally useless. But lets just say that my usefulness is severely impaired by this damned condition. Took them all, but I don’t think it was ever in combination with each other. Like most of the meds I have taken, they worked for a while then ZIPPO. It appears that the Paxil, Buspar, Neurontin Combo I am now on has gone ZIPPO on me. Does anyone know the efficacy of effexor for the new DMS category: Bipolar 2. Also, I see a lot of posts here about effexor withdrawal. If effexor is such a great drug, why are so many people coming off it all the time–are they going drug-free or are they looking for something better? Michael

I came off it because, for me, it didn’t work.  It had bad side effects for me, insomnia, strange dreams, elevated blood pressure.  I was fortunate that I had no trouble getting off it.  Others have had success with it, and also problems stopping. Charles
