is this a yeast infection?
Question:
i wake up every morning and my whole mouth is covered with a sticky grayish-white stuff, i dont have it when i go to bed at night but its there when i wake up in the morning. it covers the inside of my cheeks, my tongue and now my tonsils. also i woke up this week with a horrendous sore thoat (tonsil) and was wondering if yeast can cause this. also my tongue whole mouth is pretty sore all the time. i’m on proventil, serevent, flovent and singulair. how do you know if its yeast? and any good links out there with pictures? thanks for any advice, will
It sure sounds like yeast infection [candida]. You will need a prescription antifungal like Nystatin. Here are pictures: http://www.gastrolab.net/pa-047.htm Candia Oesophagitis due to Treatment with Inhalated Steroids Candidiasis (Cutaneous) To minimize future occurances, be sure to use an AeroChamber spacer with your Flovent MDI; rinse and gargle after inhaling and drink a glass of water to wash residue down. Ellis
Response:
Since you’re on inhaled steroids and have these symptoms, it’s a pretty good bet that you have thrush. This is a fungal infection that can be fought with Nystatin mouthwash. But why guess? It’s worth a visit to your doctor. Sounds like it to me… It is very painful I hear… I’ve seen it only on my son so I couldn’t attest to that. I would check it out with your doctor…there are some very good medications to get rid of it… – Hide quoted text — Show quoted text – i wake up every morning and my whole mouth is covered with a sticky grayish-white stuff, i dont have it when i go to bed at night but its there when i wake up in the morning. it covers the inside of my cheeks, my tongue and now my tonsils. also i woke up this week with a horrendous sore thoat (tonsil) and was wondering if yeast can cause this. also my tongue whole mouth is pretty sore all the time. i’m on proventil, serevent, flovent and singulair. how do you know if its yeast? and any good links out there with pictures? thanks for any advice, will

Discontinued Medication
Question:
Thanks to all who responded on my Tilade problem-especially Bill Ellis Fleemore. I have my medication on the way-thank goodness. Seems like my mail-order pharmacy had the medicine discontinued when all that happened was another company had bought out the originial manufacturers. After calling the new manufacturer, they called the pharmacy and sent the Tilade to them. I had been trying to reduce my Tilade without much success and was really wondering just how I was going to do without any. My doctor had tried to take me off of it 3 yrs ago to no avail so both he and I were quite concerned that my asthma was going to get out of control again. Now I don’t have to worry about that!- Pam in TN
Response:
Judy- I can certainly say that I did not expect to here my mail order pharmacy to say that Tilade is no longer available either. With the help of Ellis in this group, I was able to talk to the manufacturer today and found out that they have temporary stopped the old formula and are waiting for a new one that has already passed trials in Europe. So it looks like that the new version will be available in the US but they could not give me a definite date. So I am still looking for some of the old stuff and they are also helping me out there too. I have one canister left so I hope that I can get something worked out within 17 days before this one runs out.- Pam in TN
Response:
I found out this week that one of my mainstay medications is no longer available in the US thanks to that darn little law that was passed earlier regarding the propellants used in the inhalers. Well, my Tilade has bit the dust-along with my treatment plan that has had me in control since 1993! My plan was 2 puffs of Tilade 3 times a day and 2 sprays twice a day of FLovent 220. My pulmonary physician has told me that we will have to try to find another plan that will work since there is no substitution on the market for the Tilade. Pam in TN
– The drug manufacturers have enough CFC’s to last for several years. There should not be a problem getting Tilade if you check with a few pharmacies. We have plenty of it in New York. Wellington S. I have to admit that I need to grumble a little bit and felt like here was the place to do it. I found out this week that one of my mainstay medications is no longer available in the US thanks to that darn little law that was passed earlier regarding the propellants used in the inhalers. Until now, I never realized that my health would be at the hands of someone that knows absolutely nothing about me or my disease-just the "bottom" line boardroom tactics. It is frustrating enough to have to deal with the uncertainties of the disease but I was always comfortable knowing that my medications were keeping it under control. Now I have to go back to sqare one again- Pam in TN
Response:
Dosn’t Tilade and Intal have the same propellant ? So if one has been chucked shouldn’t the other one follow suit ? But I was under the understanding this was not going to happen until suitable replacements were available.

Pam in TN I forwarded your post to Francis Adams, MD, author of the comprehensive asthma book ‘The Asthma Sourcebook’. Here is his response. "Dear Ellis: Tilade definitely has not been discontinued. I think you must be right that they are bringing out a new formulation, probably with HFA instead of CFC. That means that they won’t supply any more of the current formula and will hopefully ship the new one soon. In terms of finding the original formula, I think it depends on the individual pharmacy and what they have in stock. I will try to find out more on Monday from the manufacturer. Good luck, Francis V. Singulair useful for COPD?
Question:
Is there any research indicating that Singulair can have a beneficial effect for people with Chronic Obstructive Pulmonary Disease? I’ve asked my doctor, an internist with a subspecialty in pulmonology, about this, and he was doubtful that the drug would be helpful to me. Since the various "classic" asthma meds are used in treating COPD symptoms, it seems strange that the anti-leukotrienes (and Singulair, in particular) have not been recommended by the drug mfrs. [I posted this question to this list several months ago. I'm asking it again, on the outside chance that there is possibly some new data available.] — Neal Blank http://p3.net/~nealb/
Response:
I am just taking a guess from what I know about Singulair. I am sure If I am way off base here aleast one of you will correct me. Singulair, goes after the chemical that at the root of the problem causes the bronchial spasms. By stopping the spasms there will be less inflammation, in turn lessing the asthma attacks. Bronchistis, and emphysema are due to inflamed bronchial tubes, but are not caused by the spasms that causes the inflammations of asthma, so Singulair will not help. I am not sure if I am right but, this may (or may not)help.

No I don’t think there is any data available. I note that in the UK the drug is only licensed for asthma. However, I have COPD with a ‘reversible componant’, which means that I respond to corticosteroid therapy with an improvement in PF and FEV1. I also respond to bronchodilators. Call this asthma if you like, and yes I have been perscribed Singulair and yes it seems to have definitely improved things for me. I have now just finished the first months treatment and the improvement I felt within days of taking it seems to have been maintained. Yesterday was the *first day for over ten years* that I have not used a single puff of Ventolin
If you can persuade them to let you have it I would say it is worth a try. Regards Rob Remove the spam from address if replying by email

Not wrong, but an oversimplification. Yes, LTD is a potent broncho-constrictor. BUt it appears to be having at least mild anti-inflammatory effects at least in mild to moderate asthmatics. THe reasons arent clear yet, but bronchospasm can lead to secondary release of mediators (prostaglandin D, Thromboxane A, cytokines, etc) which ARE inflammatory. If LTD even partially inhibits the release of such mediators from mast cells, eosinophils etc, it would have anti-inflammatory effects. BUT – *I* don’t know of any evidence that LTD is a precipitating agent in COPD. If it’s not a major player in that disease, SIngulair/Accolate’s impact is likely to be far less than in asthma. To date, there are no scientific studies of the question. Both LTE and LTB (a leukotriene NOT targeted by Accolate/Singulair) have been shown to be elevated in COPD, but whether reducing that elevation has any effect in the disease doesnt appear to have been studied. IF LTB is importand, COPD may be better treated with Zyflo than with the LT-RAs.

The Asthma Source book–new edition?
Question:
Many thanks for the info! Will check that website right away!

Does anyone know if this publisher runs early or late on publication dates and what sort of revisions were made? I am torn between getting what is available now and waiting for the revised edition.
Publication date for books are about as reliable as release dates for computer software. For both books and software I have taken the attitude: "I’ll believe it when I see it on the shelf."
Response:
When I went to my local bookstore to buy The Asthma Sourcebook (right title?) which I have often seen highly praised in this group, I was told a revised edition is due out in November of 1998–about 4 months down the road. Does anyone know if this publisher runs early or late on publication dates and what sort of revisions were made? I am torn between getting what is available now and waiting for the revised edition.

The author of ‘The Asthma Sourcebook’, Francis Adams, MD, has a web site. His email address is there. You could try emailing him with that question. Let us know what he says. http://home.earthlink.net/~francisva/news.html The Asthma Sourcebook News The existing book is copyright ‘96 and seems consistent with the ‘97 Expert Panel Report 2 in most respects. I doubt the updates would be substantial–lists of new drugs released like Pulmicort and Singulair. But you can pick up some of this info from his website. Cost of existing book is only $16 (less than a canister of Ventolin) and some stores discount, like amazon.com His web site has a link that takes you to amazon.com Ellis

Suddenly shaking hands after months on Adderall
Question:
– Hide quoted text — Show quoted text – Hello all, After taking Adderall XR 20mg for over six months with no side effects (other than some weight loss), I’ve developed shaking hands over the last month. I was thinking maybe I got a bad batch of Adderall from the pharmacist last time, but this new batch is having the same effect. My doctor has no explanation. The only change is I’ve started taking Prozac each day, but my doc says this should have no impact on my symptoms. Any thoughts are greatly appreciated! -N2 Strange… And it isn’t parkinsons? What does your doctor tells you??? no explenation? Hmmmm…my essential tremor is reduced when I am taking methylphenidate (concerta, ritalin)…I wonder if there is a connection? It’s the strangest thing I ever heard. Because methylphenidate isn’t the same as L-dopa and it isn’t effective against tremors from parkinsons. So it’s strange…
<G I do not care if it is "coincidental"….or even placebo….as long as it works <g But, from what I understand, doesn’t methylphenidate work on the dopamine levels in the patient? Besides, parkinsons is not the same thing as essential tremor…though it does appear that having ET might increase the chances of getting parkinson’s… http://en.wikipedia.org/wiki/Essential_tremor Mine currently is only in my hands (as is my mother’s)…my grandfather’s was in in hands, his arms, and showed up in his gait as well…the weird thing is, it shows up when I am "at rest," but when I do something with my hands (such as play guitar), I do not tremble as much (i.e,. when my hands are resting against the guitar, they might tremble a bit, but when then are pressed down to hold a chord, the whole guitar is not shaking, and the hand stops shaking). — Buny " Nobody realizes that some people expend tremendous energy merely to be normal." ~ Albert Camus

Concerta and increasing anxiety
Question:
– Hide quoted text — Show quoted text -I am hoping someone may have some suggestions/advice for me. I have been diagnosed with ADD for which I take Concerta (36 mgs at 6:00 am). This has helped me quite a bit. I however start to feel tense and anxious in the afternoon which will build up until I take 100 mg Seroquel about 1/2 hour before sleeping. I work in the computer field which I enjoy very much. I can’t seem to let my work go and am constantly studying and extremely curious about computers/networks in general. It seems I am always thinking out scenarios in my head about various ways to do things. The problem is that this compulsive thinking is leading to my anxious state. My dr. has tried me on numerous meds such as paxil, zoloft, effexor, luvox, depakote, lithium, remeron, wellebutrin, etc. I am not depressed, I just am (and have always been) somewhat hyper and cannot relax. It seems any med that works with serotonin just gives me bad headaches. I respond easily to Klonipin, not so well with Antivan. I guess I have ADD and am somewhat obsessive/compulsive. Erv

Question:
I just started to take 75 mg effexor xr, since the prozac i use to take, wasn’t working very well anymore. How is this anti-depressant like? Is it better than the older SSRIs? Sometimes I feel strange while I am on it. Does this go away with time? Are there more or less sexual side effects with this one compared to SSRIS like prozac and zoloft? Thanks for any help

I am now on 150 mg of Effexor, but it is only temporary. I have been on 75 mg since a couple of years ago, and there is no side effect which gives me problems. I am also on Risperdal, and recently my pdoc changed it for Zyprexa and I noted an increase of my sexual libido, so I am not sure if Effexor is lowering my libido. What I can say is that when I was married I didn’t find I needed more sexual drive than I actually had. Hi Keith. I deal with a difficult to treat depression and after trying several ADs I tried Prozac and found it helped me quite a bit for nearly two years. At least until I got hit with the infamous "Prozac poop-out". Maybe that is what hit you as well? Anyway, when it stopped working and sent me into one of the worst depressions I can remember my pdoc put me on Effexor XR. After building up to 150mg over a couple of weeks I actually started feeling better. I continued to improve and was beginning to think this may be a miracle med for me. It helped my depression much like Prozac had at first but seemed to somehow be ’smoother’ in the way it pulled me out of depression without some of the side effects I sometimes had on Prozac. That is until I started feeling not so good physically but I wasn’t sure what was wrong. I eventually had my blood pressure checked and found that it was way too high. (like 180/205) I ended up back at the pdocs and also seeing my regular medical doctor. I had read that Effexor could raise blood pressure in some people. Guess I’m one of the lucky ones. To be fair I had previously had some minor blood pressure problems that had been treated and corrected. I also have a family history of blood pressure/heart problems. I had told my pdoc about this but he said the chances of this causing me such problems were slim. I am not trying to scare you about Effexor because I think it is a great med and I wish I could stay with it. But I did want to let you know to at least be aware of this possibility and if you are not sure have your blood pressure checked once in awhile. As far as treating depression goes, this helped me a lot with very few other side effects. Good luck and I hope it helps you as much as it helped me only without the blood pressure problem! Bonnie

Effexor withdrawal, it's electrifyin'!
Question:
I’ve been on Effexor XR for a little over a month now. I started with three days at 30-odd mg and then eleven days at 75mg, and have been at 150mg for the two weeks or so after that. For shits and giggles, I decided not to take my fat pillie for just one day. Well, I made it through my day without feeling too shabby. I learned how to drive a manual transmission and was otherwise productive. Then, came the night. Every time I fell asleep, I’d get this strong feeling that an electrical pulse was being sent into my back and neck. I’d fall asleep, feel like I was shocked a few times, then wake up and ask myself, "What the hell was that?" These weren’t mild sensations. Rather, it felt like the work of a cheap stun-gun or one of those electric weight-loss belts cranked up to eleven. Now, when my friend had told me he went through some bad times coming off of Effexor, I hadn’t imagined anything like this. I’ve never had such a strong and violent response from simply NOT taking something. It seems especially odd because I’ve only been on the stuff for a month and change. But now that I’ve read more about Effexor, I know it can only get worse from here. I don’t have a point really, and I don’t have a moral. I have a definite problem with depression, and I need something to help me operate on a socially functional level, but now I’m a little scared of what the future has in store for me if I ever, heaven forbid, go longer than one day without my fix. I think I ought to force myself off of this drug ASAP and save myself from a possibly much wackier and even more extreme withdrawal process months or years down the line. Oh well, at least it doesn’t make me impotent, right? -Jon

If you punch in "effexor" on yahoo, you will get some links to sites that might offer you some help on xr withdrawal. There is an anti-nausia drug reported to help some patients ease off this drug more effectively. If you have found anymore help for withdrawal, please let us know. More and more people are reporting this same difficulty of going through withdrawal "hell" from effexor xr and have had to return taking it again just to function. Wyeth, the manufacturer knows about this and have posted more info on their site about tapering off this med. Hope this helps. Thanx Steveb

-Jon

I’d fall asleep, feel like I was shocked a few times, then wake up and ask myself, "What the hell was that…
Other people have reported "shocks" when withdrawing from Effexor and other antidepressants. I quit Effexor XR 150mg cold turkey 1.5 weeks ago because my blood pressure was dangerously high. So far I have not experienced ANY withdrawal symptoms. My blood pressure is a bit lower but it may be because I also laid off most of the caffeine.

I was on Effexor XR 150mg for over a year, and suffered no memorable withdrawal symptoms when I d/c’ed. Of course, I did taper off rather than going cold turkey, so maybe that helped. Over a year later, I had another run-in with your friend and mine, the big "D", and now I’m on Zoloft 100mg. From my personal experience, I would recommend either of these ADs to anyone, but would also suggest that they follow their doc’s instructions carefully. Andy

in general when stopping medication cold turkey one should expect withdrawal symptoms i have heard a lot of complaints from people who have stopped taking their medication abruptly…. however the simple solution to this is to not stop taking your medication abruptly
if anyone has tapered off and still found themselves unable to escape severe withdrawal symptoms, i would like to hear it

I tapered off Effexor XR with what seemed to be an entirely reasonably schedule, from 225 mg/day to 150 for a week, 75 for a week, 37.5 for a week, then half of that for a few days. Have been experiencing all the symptoms mentioned below ever since, about 2 weeks.

Hi Charles… I don’t like it. I’m sure…. This form was a ghost. Aline – Hide quoted text — Show quoted text – I have a question for you. Someone have an auditive or visual hallucination with Paxil or Effexor? Aline With Effexor I had a variety of symptoms, some could be classed as visual hallucinations. To me, it was more like dream state while I was awake, or awake state while I thought I was asleep. It also didn’t work on my depression, so I quit. No experience with Paxil. I have had what I think of as break-through dreams while awake on almost all the ADs I have used, usually within the first few weeks, and that goes away with time.

Hi, Sandi…. Glad to know I’m not the only cyber junkie here……:)) I spend way too much time on this computer….In a way, it is an escape for me…. Hi guys, I need some advice I have in this group for three weeks now and you guys have been all great with your support and I truly thank you. My problem is that I have been feeling good with taking 50mg of zoloft in fact this is the first week that I have been out of the house, driving and have some fun. The only thing is that yesterday I was having alot of short of breath (it went away later that day). Last night I started with this weird feeling in the middle of my chest like I ate too much or something, but it felt like I was going to burst. It was a tightening I guess that how I want to explain it well, anyway it started scaring me I thought I was going to have a heart attack or something. My husband who I might say is great about all this PA, told me to try to relax, but it was so hard because I was trying to go to sleep and I was really scared. I gon’t up this morning and I am ok now, does anyone know if they ever had this or is it a side effect from zoloft? Sorry that this is a long letter. and Thanks for being there. Also, a little LOL I had withdraw from not being able to get on my computer yesterday, My son took the mouse to work with him.
